目的：分析前臂皮瓣与胸大肌皮瓣在口腔癌手术缺损修复中的应用，探讨其在临床上的应用效果。方法：选取我院2008年10月~2012年11月收治的84例在口腔癌根治性切除术后接受口腔癌手术缺损修复的患者，根据移植部位不同分为前臂组和胸大肌组，每组各42例。前臂组和胸大肌组分别采用前臂桡侧皮瓣、胸大肌皮瓣修复口腔癌术后口腔颌面部缺损，术后观察比较两组患者的皮瓣成活率、并发症及外形恢复情况。结果：前臂组患者的皮瓣成活率与胸大肌组比较不存在显著差异(P>0.05)，两组皆发生不同程度的并发症及外形恢复问题(P>0.05)，但均未对正常的生活造成严重的影响。结论：在口腔癌手术缺损修复中采用前臂皮瓣与胸大肌皮瓣效果较好，适用于不同的缺损类型，均在临床上值得推广。
목적：분석전비피판여흉대기피판재구강암수술결손수복중적응용，탐토기재림상상적응용효과。방법：선취아원2008년10월~2012년11월수치적84례재구강암근치성절제술후접수구강암수술결손수복적환자，근거이식부위불동분위전비조화흉대기조，매조각42례。전비조화흉대기조분별채용전비뇨측피판、흉대기피판수복구강암술후구강합면부결손，술후관찰비교량조환자적피판성활솔、병발증급외형회복정황。결과：전비조환자적피판성활솔여흉대기조비교불존재현저차이(P>0.05)，량조개발생불동정도적병발증급외형회복문제(P>0.05)，단균미대정상적생활조성엄중적영향。결론：재구강암수술결손수복중채용전비피판여흉대기피판효과교호，괄용우불동적결손류형，균재림상상치득추엄。
Objective:Analysis the clinical efficacy of radial forearm free flap and pectoralis major myocutaneous flap in oral cancer reconstruction, to investigate the clinical efficacy.Methods:84 patients accepted oral cancer reconstruction after oral cancer radical resection that admitted in our hospital from October 2008 to November 2012 were divided into the radial forearm free flap groups and the pectoralis major myocutaneous flap group according to the transplantation site, n=42 cases. The radial forearm free flap groups and the pectoralis major myocutaneous flap group were used radial forearm free flap and, pectoralis major myocutaneous flap to repaired the oral and maxillofacial defect, postoperative observation compared two groups of patients, complications, the situation of shape recovery and the survival rate of flaps.Results:The survival rate of flaps of the radial forearm free flap groups compared with the pectoralis major myocutaneous flap group there is no significant difference (P>0.05), both groups have the varying degrees of complications, shape recovery problems(P>0.05), but are not severely affected normal life.Conclusion:Both radial forearm free flap and pectoralis major myocutaneous flap show a good effect in oral cancer reconstruction, suitable for different defect types, worthy of clinical application.
