目的:探讨儿童骨干续连症所致四肢畸形的手术方法和疗效,总结手术治疗的安全性、疗效及注意事项。方法:2005年6月至2010年6月本院收治的12例因多发性骨软骨瘤导致前臂畸形及股骨短缩畸形患儿,采用尺骨截骨Orthofix技术逐渐延长尺骨,矫正弯曲,同时牵引桡骨小头复位。股骨短缩患者采用股骨截骨延长,术后均通过随访,评价患肢功能和外观改善情况。结果:10例前臂短缩、弯曲得到矫正,腕关节尺偏纠正,2例股骨软骨瘤短缩患肢不等长外观纠正外观满意。前臂旋转功能较术前改善,旋前平均增加10°~16°,旋后活动度平均增加11°~14°。8例桡骨头脱位复位满意,1例桡骨头脱位未复位。结论:采用单臂外固定架骨延长术治疗多发性骨软骨瘤所致的四肢畸形的技术简便可靠,并发症少,在治疗上还要注意选择恰当的手术时机。
目的:探討兒童骨榦續連癥所緻四肢畸形的手術方法和療效,總結手術治療的安全性、療效及註意事項。方法:2005年6月至2010年6月本院收治的12例因多髮性骨軟骨瘤導緻前臂畸形及股骨短縮畸形患兒,採用呎骨截骨Orthofix技術逐漸延長呎骨,矯正彎麯,同時牽引橈骨小頭複位。股骨短縮患者採用股骨截骨延長,術後均通過隨訪,評價患肢功能和外觀改善情況。結果:10例前臂短縮、彎麯得到矯正,腕關節呎偏糾正,2例股骨軟骨瘤短縮患肢不等長外觀糾正外觀滿意。前臂鏇轉功能較術前改善,鏇前平均增加10°~16°,鏇後活動度平均增加11°~14°。8例橈骨頭脫位複位滿意,1例橈骨頭脫位未複位。結論:採用單臂外固定架骨延長術治療多髮性骨軟骨瘤所緻的四肢畸形的技術簡便可靠,併髮癥少,在治療上還要註意選擇恰噹的手術時機。
목적:탐토인동골간속련증소치사지기형적수술방법화료효,총결수술치료적안전성、료효급주의사항。방법:2005년6월지2010년6월본원수치적12례인다발성골연골류도치전비기형급고골단축기형환인,채용척골절골Orthofix기술축점연장척골,교정만곡,동시견인뇨골소두복위。고골단축환자채용고골절골연장,술후균통과수방,평개환지공능화외관개선정황。결과:10례전비단축、만곡득도교정,완관절척편규정,2례고골연골류단축환지불등장외관규정외관만의。전비선전공능교술전개선,선전평균증가10°~16°,선후활동도평균증가11°~14°。8례뇨골두탈위복위만의,1례뇨골두탈위미복위。결론:채용단비외고정가골연장술치료다발성골연골류소치적사지기형적기술간편가고,병발증소,재치료상환요주의선택흡당적수술시궤。
Objective:To explore the surgery method and effect of treating diaphyseal aclasis limb deformities in children, and to summarize the safety, efficacy, and precautions of surgical treatment.Methods:12 cases caused due to multiple osteochondroma forearm deformity and shortening of the femoral deformity in children was admitted to our hospital from June 2005 to June 2010, using the ulnar osteotomy Orthofix technology and gradually extended the ulna, correcting of the bending, traction radial head reset. The shortening of the femur patients using femoral osteotomy extend method.To evaluate the improvement of limb function and appearance through postoperative follow-up.Results:In 10 cases of forearm shortening,the bending have been corrected, wrist ulnar deviation was corrected, in 2 cases femoral shortening, the appearance and length of the femoral was corrected satisfactorily. Compared with preoperative, the forearm rotation was improved. Increased by an average of 10 ° to 16 ° pronation, and 11 ° to 14 °supination. 8 cases of radial head dislocation was reset satisfied and one case was not reset.Conclusion:Unilateral external fixator lengthening surgery method in the treatment of multiple osteochondroma caused limb deformities technology is simple, reliable, and withfewer complications, and the proper timing of surgery in the treatment must be selected.
