目的：研究弥漫性轴索损伤（diffuse axonal injury，DAI）患者相应观测指标对预后的影响，筛选出影响DAI预后的相关因素，为提高DAI的治疗水平提供临床依据。方法对121例DAI患者资料进行回顾性分析，研究预后与GCS评分、脑肿胀、瞳孔变化、蛛网膜下腔出血的相关性，脑肿胀患者进一步研究颅内压情况与预后关系，并进行相应治疗探讨。结果入院时GCS、瞳孔异常、脑肿胀情况与患者预后有关，脑肿胀患者颅内压增高程度与预后有关，蛛网膜下腔出血与预后无关。结论GCS越高、无瞳孔变化、无脑肿胀患者预后更好，脑肿胀引起颅内压增高的程度与预后负相关。
목적：연구미만성축색손상（diffuse axonal injury，DAI）환자상응관측지표대예후적영향，사선출영향DAI예후적상관인소，위제고DAI적치료수평제공림상의거。방법대121례DAI환자자료진행회고성분석，연구예후여GCS평분、뇌종창、동공변화、주망막하강출혈적상관성，뇌종창환자진일보연구로내압정황여예후관계，병진행상응치료탐토。결과입원시GCS、동공이상、뇌종창정황여환자예후유관，뇌종창환자로내압증고정도여예후유관，주망막하강출혈여예후무관。결론GCS월고、무동공변화、무뇌종창환자예후경호，뇌종창인기로내압증고적정도여예후부상관。
Objective To analyze the influence of corresponding clinical observations on the prognosis in patients with DAI,and to select the indices that can affect the prognosis to improve the therapeutic level of DAI patients.Method Clinical data of 121 DAI patients were retrospectively analyzed to identify the relationship between the indices and outcomes.Result The indices,including GCS,abnormal pupil,ICP and DBS were correlated with outcomes.Subarachnoid hemorrhage were not combined with therapeutic efifcacy.Conclusion The patients with higher GCS,combined with none DBS,with abnormal pupil size and reaction,with light degree of ICP in DBS may have better outcome.
