目的:探讨超声引导下冷循环微波刀在肝癌治疗中的近期疗效。方法对我院2008年3月至2010年4月收治的81例患者共112个病灶进行冷循环微波刀治疗,术前超声造影检查(CEUS)及微血管成像(MFI)并测量肿瘤大小,术后超声造影(CEUS)观察肿瘤灭活情况及肿瘤大小以及AFP变化情况,随访1、2、3年生存率。结果本组术前血清AFP(429±29.8)μg/L,术后1个月降低为(234±22.3)μg/L;病人术前病灶平均直径(50.2±11.3)mm,术后1个月病灶平均直径为(38.2±6.9)mm(P<0.05);术后3个月病灶完全消失10例计12个病灶,无明显变化者8个病灶,余者均有不同程度缩小。术后1个月超声造影显示97个(86.6%)病灶呈无血流灌注、边界清晰的无增强状态,15个病灶部分边缘增强。结论超声引导下冷循环微波刀联合使用超声造影创治疗肝癌是一种十分有效的方法,作为微创治疗肝癌手段值得推广。

Objective To investigate the short-term outcome of ultrasound-guided psychro-circulation percutaneous microwave coagulation therapy in treatment for liver cancer. Methods From March 2008 to April 2010, 81 liver cancer patients with 112 lesions underwent psychro-circulation percutaneous microwave coagulation therapy (PPMCT) in our hospital. Before operation, the contrast-enhanced ultrasound (CEUS) and the micro flow imaging (MFI) as well as the measurement of tumor size were taken. After operation, the inacti-vation of tumor and the changes in tumor size as well as serum AFP were measured. The survival rates of the patients after 1year, 2 years and 3 years were followed up and recorded. Results After one month treatment, serum AFP was significantly decreased from (429 ± 29.8) μg/L to (234 ± 22.3) μg/L(P<0.01) ; the average lesion diameter was also significantly reduced from (50.2 ± 11.3) mm to (38.2 ± 6.9) mm (P<0.05). Three months later, 12 lesions in 10 cases was completely disappeared; no significant changes were found in 8 le-sions, and the remaining lesions were reduced in various degree. After one month, CEUS showed 97 lesions (86.6%) had clear boundary without enhanced status, portion edge enhancement was found in 15 lesions. Conclusion Ultrasound-guided psychro-circulation percutaneous microwave coagulation therapy associated with CEUS treating liver cancer is a very effective minimally invasive method.
