提目的:探讨应用高粘度骨水泥对急性重度骨质疏松椎体压缩性骨折(Osteoporotic vertebral compression fracture ,OVCF )行经皮椎体成形术(Percutaneous vertebroplasty ,PVP)的临床疗效。方法:收集2005年2月至2010年8月急性OVCF患者36例,体位复位后应用高粘度骨水泥行经皮椎体成形术治疗,手术前后进行视觉模拟疼痛评分(VAS )、伤椎高度与后凸畸形的X线片测量。结果:全部病例手术均安全完成,术中骨水泥注入量平均3.0ml ,术后平均随访16.4(12至30)个月。术后 VAS评分较术前改善(P<0.05),椎体前缘、中线高度及后凸畸形较术前明显恢复(P<0.05)。4例(11.1%)出现骨水泥渗漏,但无临床症状及体征。结论:应用高粘度骨水泥经皮椎体成形术结合体位复位及是治疗急性OVCF可行及有效的方法。

Objective :To evaluate the clinical efficacy of percutaneous vertebroplasty (PVP ) with high viscosity bone cement in treating acute osteoporotic vertebral compression fracture (OVCF) .Methods :From February 2005 to August 2010 ,thirty -six patients with acute OVCF were treated by postural reduction and PVP with high viscosity bone cement .Pain relief ,restoration of vertebral height and kyphosis’correction were evaluated by comparison before and after operation using visual analogue scale (VAS) ,radiography .Results :All cases underwent operation successfully with average injected bone cement 2 .8ml .The mean time of follow -up was 16 .4months(12-30months) .The patients’VAS scores were significantly improved after operation (P<0 .05) .The height restoration of vertebral anterior border and midline and the correction of kyphosis were significantly improved (P< 0 .05 ) .Cement leakage was found in 4 cases (11 .1% ) without any clinical symptoms and signs .Conclusion :PVP with high viscosity bone cement and postural reduction are effective and relatively safe for the treating acute OVCF .
