目的探讨临床分离酵母菌分子分类方法和药物敏感性特征.方法临床收集复发性外阴阴道假丝酵母菌病的93株酵母菌,应用法国生物梅里埃酵母菌药敏试剂盒ATBTMFUNGUS3进行体外药物敏感性试验,以核糖体基因26S rDNA D1/D2区的序列分析为依据研究其菌种的分子分类方法.结果 93株酵母菌分为光滑假丝酵母菌(38/93)、白色假丝酵母菌(23/93)、近平滑假丝酵母菌(16/93)、似平滑假丝酵母菌(9/93)等9个种11个基因型.其中有18株为敏感株,3株完全耐药.5种常用抗真菌药物的敏感性是5FC 83.87%,AMB 93.55%,FCA 27.96%,ITR 21.51%,VRC 43.01%.结论基于核糖体基因的分子分类法可以准确鉴定酵母菌临床分离株,复发性外阴阴道假丝酵母菌病菌种呈现以光滑假丝酵母菌为主的非白色假丝酵母菌占75.27%的分布特征.93株酵母菌对AMB,5FC的敏感性高于三唑类药物,三唑类药物中ITR耐药率高于VRC,FCA.药物敏感性与菌种有关.

Objective To investigate the classification method of saccharomycetes collected in clinic and the feature of sensitivity to drugs. Method 93 strains of saccharomycetes causing recurrent vulvovaginal candidiasis were collected in clinic, France merry biological medicine sensitive kit, ATBTM FUNGUS3 ,was used for the drug sensitivity test, and the molecular classification method for the saccharomycete was studied based on ribosome gene 26S rDNA D1 / D2 sequence analysis. Results 93 strains of saccharomycetes were classified as Candida glabrata ( 38/93 ), Candida albicans ( 23/93 ), Candida parapsilosis ( 16/93 ), Candida glabrata-like saccharomycete (9/93) and they included nine species and 11 gene types. 18 Strains of them were sensitive and 3 strains were completely resistant to the drugs. The sensitivity test for 5 frequently used antimycotic drugs showed that 5 FC was 83.87% , AMB was 93.55% , FCA was 27.96% , ITR was 21.51% and VRC was 43.01%. Conclusion Based on the molecular taxonomy of ribosome gene, it is possible accurately to identify the saccharomycetes collected in clinic. The feature of saccharomycetes causing recurrent vulvovaginal eandidiasis should be primarily Candida glabrata and Candida albicans accounts for 75.27%. The sensitivity of 93 strains of saccharomycetes to AMB and 5 FC should be higher than that of them to the triazole drugs. Among the triazole drugs, the resistant rate of saccharomycetes to VRC should be higher than that of them to FCA and ITR. It is deduced that drug sensitivity could be associated with strains.
