目的:探讨小儿先天性心脏病合并不同程度肺动脉高压(PAH)时,血清 BNP 与右心室 FAC 和 Tei 指数的相关性。方法:CHD 患儿50例,分为无 PAH 组(n=12)和 PAH 组(n=38)。PAH 组再按肺动脉压力程度分为轻度组(n=12)、中度组(n=14)、重度组(n=12)。同时选择15名健康体检者作为正常对照组。采用 GE logic7检查肺动脉和三尖瓣口血流频谱。结果:①右室 Tei、血清 BNP 在 PAH 组明显高于无 PAH 组和正常对照组(P<0.05),且随着肺动脉压力程度加重,Tei 指数和血清 BNP 值上升。②血清 BNP 与右室 Tei 指数显著正相关,与右心室 FAC 呈显著负相关。结论:右室 Tei 指数和血清 BNP 均能很好的反映右心室功能。

목적:탐토소인선천성심장병합병불동정도폐동맥고압(PAH)시,혈청 BNP 여우심실 FAC 화 Tei 지수적상관성。방법:CHD 환인50례,분위무 PAH 조(n=12)화 PAH 조(n=38)。PAH 조재안폐동맥압력정도분위경도조(n=12)、중도조(n=14)、중도조(n=12)。동시선택15명건강체검자작위정상대조조。채용 GE logic7검사폐동맥화삼첨판구혈류빈보。결과:①우실 Tei、혈청 BNP 재 PAH 조명현고우무 PAH 조화정상대조조(P<0.05),차수착폐동맥압력정도가중,Tei 지수화혈청 BNP 치상승。②혈청 BNP 여우실 Tei 지수현저정상관,여우심실 FAC 정현저부상관。결론:우실 Tei 지수화혈청 BNP 균능흔호적반영우심실공능。
Objective:Study on the correlation of right ventricular Tei index,fractional area change (FAC) and serum BNP in children with congenital heart disease(CHD) in different level of pulmonary hypertension (PH). Methods:A total of 50 children with CHD and 15 age-matched healthy children were involved in this research. The patients were divided into nonPH group(CHD without PH,n=12)and PH group(CHD with PH, n=38). The PH group were divided into mild(n=12), moderate (n=14) and severe(n=12) subgroups according to pulmonary artery pressure. The pulse Doppler flow of the pulmonary valve and tricuspid were examined by GE logic 7. Two-dimensional standard apical four-chamber image of consecutive three cardiac cycles were preserved. Results:① Right ventricular Tei and BNP in PH group were significantly higher than those in nonPH group and controls(P<0.05); they decreased with the increase of PH . ②BNP correlated positively with Tei index and negatively with FAC. Conclusion:Right ventricular Tei index and BNP can properly assess right ventricular function.
