目的:探讨长期使用双膦酸盐是否会增加非典型股骨骨折的发生风险。方法计算机检索相关中英文数据库,采用循证医学方法,对近年来发表的有关双膦酸盐与非典型股骨骨折关系的文献进行荟萃分析;对收集的资料,采用Stata 12.0软件进行Meta分析。结果共纳入6篇英文文献,共686929例患者;Meta结果显示,应用双膦酸盐类药物明显增加患者非典型股骨骨折的发生风险,差异具有统计学意义(RR=1.55,95% CI 0.94~2.16,P<0.05),应用双膦酸盐类药物>5年及以上明显增加患者非典型股骨骨折的发生风险,差异具有统计学意义(RR=1.54,95%CI 1.16~1.95,P<0.05)。结论根据目前现有临床研究证据,长期使用双膦酸盐类药物可明显增加非典型股骨骨折发生风险,在临床工作中需谨慎使用。

Objective To investigate whether long-term use of bisphosphonates may increase the risk of atypical femur fractures(AFF). Methods The relevant databases of English and Chinese were searched by us. According to evidence-based medicine, the published literature in recent years about the relationship between bisphosphonates and AFF was meta-analysis. The collected data was analyzed by Stata 12.0 Meta-analysis software. Results A total of six English literature and 686 929 cases of patients were included in this study. Meta showed that application of bisphosphonate drugs significantly increased the risk of patients with AFF, the difference was statistically significant (RR=1.55, 95% CI 0.94-2.16, P<0.05), application of bisphosphonate drugs>5 years and above significantly increased the risk of patients with AFF, the difference was statistically significant (RR=1.54, 95%CI 1.16-1.95, P<0.05). Conclusion According to clinical studies currently available evidence, long-term use of bisphosphonate drugs can significantly increase the risk of AFF, which in clinical work with caution.
