目的:探讨自行新研制的镁合金可吸收肋骨髓内钉在肋骨骨折内固定中的生物力学性能,并与进口的聚左旋乳酸可吸收肋骨髓内钉内固定方法进行比较,为临床应用提供科学依据。方法采集30根成人第5肋骨侧肋新鲜标本,行侧肋腋中线处肋骨骨折模型,分别采用AZ31B镁合金可吸收肋骨髓内钉(AZ31B组)、聚左旋乳酸可吸收肋骨髓内钉刚子(PLLA组)两种内固定方法固定,运用实验应力分析手段测试各组试件的生物力学特性,并进行比较。另设正常肋骨标本组以作对照。结果(1)三点弯曲强度测量,AZ31B组接近正常标本(P>0.05),而AZ31B组与PLLA组相差显著(P<0.05);(2)扭转强度测量表明,AZ31B与正常标本无显著性差异,而比PLLA固定优越(P<0.05);(3)拉伸试验表明,AZ31B固定抗拉强度优于PLLA内固定(P<0.05);抗拔力AZ31B明显优于PLLA内固定(P<0.05)。结论镁合金可吸收肋骨髓内钉在强度和抗拉伸强度上均明显优于聚左旋乳酸可吸收肋骨髓内钉内固定,是肋骨内固定的理想固定材料。

목적:탐토자행신연제적미합금가흡수륵골수내정재륵골골절내고정중적생물역학성능,병여진구적취좌선유산가흡수륵골수내정내고정방법진행비교,위림상응용제공과학의거。방법채집30근성인제5륵골측륵신선표본,행측륵액중선처륵골골절모형,분별채용AZ31B미합금가흡수륵골수내정(AZ31B조)、취좌선유산가흡수륵골수내정강자(PLLA조)량충내고정방법고정,운용실험응력분석수단측시각조시건적생물역학특성,병진행비교。령설정상륵골표본조이작대조。결과(1)삼점만곡강도측량,AZ31B조접근정상표본(P>0.05),이AZ31B조여PLLA조상차현저(P<0.05);(2)뉴전강도측량표명,AZ31B여정상표본무현저성차이,이비PLLA고정우월(P<0.05);(3)랍신시험표명,AZ31B고정항랍강도우우PLLA내고정(P<0.05);항발력AZ31B명현우우PLLA내고정(P<0.05)。결론미합금가흡수륵골수내정재강도화항랍신강도상균명현우우취좌선유산가흡수륵골수내정내고정,시륵골내고정적이상고정재료。
Objective To study the biomechanical characteristics of a new magnesium AZ 31B alloy intr-amedullary nail for treatment of rib fracture .This study was made to provide some biomechanical evidences for better clinical operation .Methods Thirty adult ribs were divided into 3 groups randomly ,and made models for the mid-dle fractures.The fractured ribs were fixed with AZ31B alloy intramedullary nail (AZ31B) and Poly-L-lactic acid intramedullary nail (PLLA),respectively.In addition,healthy ribs were taken as control group .The biomechanical test was performed and the stress was analyzed . Results ( 1 ) The bending strength of internal fixation with AZ31B was close to the control group (P>0.05);there was remarkable difference between PLLA group and AZ 31B group (P<0.05).(2) The torsion strength of internal fixation with AZ31B was close to the normal group(NS), but stronger than the PLLA group(P<0.05); (3) The tensile strength of AZ31B internal fixation was larger than the PLLA group(P<0.05);the anti-putout power of internal fixation with AZ31B was remarkably larger than the PLLA group(P<0.05).Conclusion The bending strength,tensile strength and anti-putout power of AZ31B are larger than those of PLLA.The internal fixation with AZ31B is an ideal method for treating rib fracture .
