目的：对临床常用的髌骨骨折内外固定方法的疗效及手术依从性进行分析，证实抓髌器外固定的临床优势。方法将我院2007年1月～2012年12月间手术治疗的128例髌骨骨折患者随机分为2组，每组研究对象64例。一组使用抓髌器外固定，为观察组；一组使用克氏针张力内固定，为对照组；治疗后对两组研究对象的骨折愈合、并发症、关节功能、患者满意率等临床数据进行比较及统计学分析。结果观察组62例、对照组58例骨折愈合良好，观察组愈合优良率（96.9％）高于对照组（90.6％），且具有显著差异性（ P＜0.05）；在感染发生率及关节功能的比较中，观察组均优于对照组，且具有显著差异性（ P ＜0.05）；观察组患者满意率为98.4％，均优于对照组，且具有显著差异性（ P＜0.05）。结论抓髌器外固定在髌骨骨折治疗中的疗效及依从性较克氏针张力内固定具有显著优势，尤其在髌骨粉碎性骨折中优势更加明显，适于临床推广使用。
목적：대림상상용적빈골골절내외고정방법적료효급수술의종성진행분석，증실조빈기외고정적림상우세。방법장아원2007년1월～2012년12월간수술치료적128례빈골골절환자수궤분위2조，매조연구대상64례。일조사용조빈기외고정，위관찰조；일조사용극씨침장력내고정，위대조조；치료후대량조연구대상적골절유합、병발증、관절공능、환자만의솔등림상수거진행비교급통계학분석。결과관찰조62례、대조조58례골절유합량호，관찰조유합우량솔（96.9％）고우대조조（90.6％），차구유현저차이성（ P＜0.05）；재감염발생솔급관절공능적비교중，관찰조균우우대조조，차구유현저차이성（ P ＜0.05）；관찰조환자만의솔위98.4％，균우우대조조，차구유현저차이성（ P＜0.05）。결론조빈기외고정재빈골골절치료중적료효급의종성교극씨침장력내고정구유현저우세，우기재빈골분쇄성골절중우세경가명현，괄우림상추엄사용。
Objective To analyze the efficacy and compliance of internal fixation and external fixation in the treatment of patellar fracture and to verify the clinical advantage of external fixation for patellar fracture .Meth-ods A total of 128 cases of patellar fracture who received operation from Sep .2011 to Sep.2012 were randomly di-vided into 2 groups (64 cases per group).Patients who received external fixation with patellar device were taken as observation group ,and patients who received internal fixation with Kirschner wire were taken as control group .Frac-ture healing,complications,joint function and satisfaction rates were compared and statistically analyzed .Results Good fracture healing was observed in 62 cases in the observation group ,and 58 cases in the control group .The ex-cellent and good rate of fracture healing was 96.9%in the observation group ,while 90.6%in the control group ,with significant difference(P<0.05);the infection rate was lower and joint function was better in the observation group than the control group , with significant difference ( P<0.05 ); the satisfaction rate of observation group was also higher in the observation group (98.4%),which also has significant difference (P<0.05).Conclusion The pa-tellar external fixation in the treatment of patella fractures can get better efficacy and compliance compared with Kir -schner wire internal fixation ,especially for patients with patellar comminuted fracture .Patellar external fixation is worth widely applying in clinical practice .
