目的研制一种辅助股骨颈空心螺钉导针植入的瞄准器,并探讨其临床应用效果。方法自行设计制造新型瞄准器并将其应用于临床。2010年1月至2011年11月共有40例股骨颈骨折患者符合入组标准,将其随机分为2组,行闭合复位空心螺钉内固定术,试验组术中使用瞄准器,对照组不用。记录并对比两组手术时间、术中出血量、穿针次数、第一枚空心钉与股骨颈冠状面中轴线的夹角、第一枚空心钉与股骨颈矢状面中轴线的夹角及空心钉与股骨距最近距离。结果两组手术时间、术中出血量差异无统计学意义( P>0.05)。试验组穿针次数少于对照组,第一枚空心螺钉与股骨颈冠状面和矢状面中轴线的夹角小于对照组,空心钉与股骨距最近距离小于对照组,差异均有统计学意义( P<0.05)。结论该瞄准器结构简单,操作方便,能有效辅助穿针,有较高的临床应用价值。

목적연제일충보조고골경공심라정도침식입적묘준기,병탐토기림상응용효과。방법자행설계제조신형묘준기병장기응용우림상。2010년1월지2011년11월공유40례고골경골절환자부합입조표준,장기수궤분위2조,행폐합복위공심라정내고정술,시험조술중사용묘준기,대조조불용。기록병대비량조수술시간、술중출혈량、천침차수、제일매공심정여고골경관상면중축선적협각、제일매공심정여고골경시상면중축선적협각급공심정여고골거최근거리。결과량조수술시간、술중출혈량차이무통계학의의( P>0.05)。시험조천침차수소우대조조,제일매공심라정여고골경관상면화시상면중축선적협각소우대조조,공심정여고골거최근거리소우대조조,차이균유통계학의의( P<0.05)。결론해묘준기결구간단,조작방편,능유효보조천침,유교고적림상응용개치。
Objective To evaluate clinical outcomes of a novel aiming device for the placement of cannulated screws guide pins in the treatment of femoral neck fractures .Methods We made a novel aiming device and applied it to clinical use .Between January 2010 and November 2011 ,there were 40 femoral neck fracture patients who were included into the study .All 40 patients were divided into two groups randomly .The aiming device was used during the surgery for patients of experimental group while the aiming device was not allowed to use during the surgery for patients of control group .Results There were no statistically significant difference in surgery time and bleed volume between two groups ( P >0.05 ) .The number of guide pin placement were significantly fewer in experimental group than control group (P<0.05),meanwhile,the angle between the first cannulated screw and the middle line of femoral neck in coronal plane , the angle between the first cannulated screw and the middle line of femoral neck in sagittal plane and the distance between cannulated screws and femoral calcar were significantly smaller in experimental group than control group ( P<0.05 ) .Conclusion The aiming device , which is simple in structure and easy to use ,could help surgeons to place cannulated screws guide pins ,and it is valuable to apply it to clinical use widely .
