目的:观察甘露聚糖肽注射液对开放性骨折患者免疫功能的影响及预防并发感染的临床效果。方法:开放性骨折患者110例随机分为试验组和对照组,对照组采用常规方式给予治疗,并根据药敏试验选择合适的抗菌药物。试验组在对照组治疗基础上静脉点滴甘露聚糖肽注射液,观察患者免疫功能指标、临床体征的变化。结果:两组患者术毕时CD3+、CD4+、CD4+/CD8+、NK细胞等指标组间无统计学差异(P>0.05)。试验组术后第4、7天CD3+、CD4+、CD4+/CD8+、NK细胞等指标较术毕时明显升高,且显著高于同期对照组(P<0.05)。试验组术后感染并发症发生率比对照组具统计学差异(P<0.05)。试验组患者平均住院时间较对照组缩短(P<0.05)。试验组患者最高体温与对照组比较差异无统计学意义(P>0.05),但退热时间较对照组明显(P<0.05)。试验组术后第4、7天临床总有效率比对照组总有效率分别增加,两组比较无统计学差异(P>0.05)。结论:开放性骨折患者应用甘露聚糖肽注射液能解除免疫抑制状态,恢复免疫功能,增强其对病原菌感染的抵抗力,促进患者创口愈合和身体康复。

목적:관찰감로취당태주사액대개방성골절환자면역공능적영향급예방병발감염적림상효과。방법:개방성골절환자110례수궤분위시험조화대조조,대조조채용상규방식급여치료,병근거약민시험선택합괄적항균약물。시험조재대조조치료기출상정맥점적감로취당태주사액,관찰환자면역공능지표、림상체정적변화。결과:량조환자술필시CD3+、CD4+、CD4+/CD8+、NK세포등지표조간무통계학차이(P>0.05)。시험조술후제4、7천CD3+、CD4+、CD4+/CD8+、NK세포등지표교술필시명현승고,차현저고우동기대조조(P<0.05)。시험조술후감염병발증발생솔비대조조구통계학차이(P<0.05)。시험조환자평균주원시간교대조조축단(P<0.05)。시험조환자최고체온여대조조비교차이무통계학의의(P>0.05),단퇴열시간교대조조명현(P<0.05)。시험조술후제4、7천림상총유효솔비대조조총유효솔분별증가,량조비교무통계학차이(P>0.05)。결론:개방성골절환자응용감로취당태주사액능해제면역억제상태,회복면역공능,증강기대병원균감염적저항력,촉진환자창구유합화신체강복。
Objective:To observe the influence of Mannatide injection on the immune function of open fracture patients and its clinical efficacy of preventing concurrent infection. Methods:110 open fracture patients in our hospital were randomly divided into test group and control group. The control group was given conventional therapy and appropriate antimicrobial based on susceptibility testing. The test group was given intravenous Mannatide infusion plus the same therapy used in the control group. Immune function indexes and the changes of clinical signs were observed. Results:There was no statistically significant difference (P>0.05) in immune indexes between the two groups when just surgery. CD3+,CD4+,CD4+/CD8+,NK cells and other immune indexes in test group were significantly increased and much higher than that of the control group in the same period when 4 and 7 days after surgery (P<0.05). The postoperative infection complication rate of the two groups were significant different (x2=4.27,P=0.039). The average stay of patients in test group was shorter than control group (P=0.017). The highest temperature of the experimental group patients compared with control group showed no significant difference (P=0.65), while fever clearance time was significantly shorter than the control group (P=0.021). The total effective rate of test group at the 4th and 7th day after the surgery was higher than control group, but there showed no significant difference.(P=0.67,P=0.25). Conclusion:Mannatide injection can relieve the immune suppression, restore immune function, enhance its resistance on pathogen infection in open fracture patients and to promote wound healing in patients and physical rehabilitation.
