目的:对肺结核耐药影响因素进行研究和分析,旨在为肺结核治疗提供可靠依据。方法:采集我院肺结核患者痰标本,并采用改良酸性罗氏培养基进行菌株培养,经TCH、PNB鉴别分型,确定137株作为药敏研究对象,对临床较常用的8种抗结核药物耐药性情况进行检测和分析。结果:137株中有108例为人结核分枝杆菌,有27例为牛结核分支杆菌,2例为非结核分枝杆菌。单一耐药情况为:初治组耐药情况为异烟肼(H)9.83%>链霉素(S)6.56%>乙胺丁醇(E)1.64%;复治组耐药情况为异烟肼(H)10.39%>利福平(R)、链霉素(S)2.6%>乙胺丁醇(E)、氧氟沙星(Ofx)1.30%,组合耐药初治与复治药物耐药具有显著差异(P<0.05)。结论:肺结核致病菌以人结核杆菌为主,临床8种常见抗结核药物均有一定耐药性,以是异烟脐、链霉素表现最为明显,初治与复治为组合耐药性的关键影响因素。

목적:대폐결핵내약영향인소진행연구화분석,지재위폐결핵치료제공가고의거。방법:채집아원폐결핵환자담표본,병채용개량산성라씨배양기진행균주배양,경TCH、PNB감별분형,학정137주작위약민연구대상,대림상교상용적8충항결핵약물내약성정황진행검측화분석。결과:137주중유108례위인결핵분지간균,유27례위우결핵분지간균,2례위비결핵분지간균。단일내약정황위:초치조내약정황위이연정(H)9.83%>련매소(S)6.56%>을알정순(E)1.64%;복치조내약정황위이연정(H)10.39%>리복평(R)、련매소(S)2.6%>을알정순(E)、양불사성(Ofx)1.30%,조합내약초치여복치약물내약구유현저차이(P<0.05)。결론:폐결핵치병균이인결핵간균위주,림상8충상견항결핵약물균유일정내약성,이시이연제、련매소표현최위명현,초치여복치위조합내약성적관건영향인소。
Objective:Factors impact resistant tuberculosis research and analysis designed to provide a reliable basis for tuberculosis treatment. Methods:137:Acquisition of tuberculosis patients sputum specimens in our hospital, and modified acid carried Roche medium, strains were cultured, TCH, PNB identify genotyping to determine drug sensitivity study, the clinical 8 anti-TB drug resistance detection and analysis. Results:137 of the 108 cases of human M. tuberculosis, 27 cases of Mycobacterium tuberculosis for cattle, two cases of non-tuberculous mycobacteria. A single drug:the naive group drug resistance to isoniazid (H) 9.83%>streptomycin (S) 6.56%>ethambutol (E) 1.64%;retreatment group resistant to isonicotinic hydrazine (H) 10.39%>rifampicin (R), streptomycin (S) 2.6%>ethambutol (E), ofloxacin (Ofx) 1.30%, the combination drug initial treatment and retreatment drugs resistance with a significant difference (P<0.05). Conclusions:The tuberculosis pathogens People Mycobacterium tuberculosis based, there is a certain resistance to common anti-TB drugs in clinical, so isonicotinoyl umbilical, streptomycin was the most obvious, initial treatment and retreatment of a combination of resistance the key factors.
