Nursing service is the one of process of evidence forming .By means of the evidence system of nursing service, the evidence during nursing process could be input to the management system .The article intro-duced the necessity and basic principle of constructing the evidence system , discussed its contents and methods , so as to guide nursing staff to collect and reserve evidence correctly , in case that the risks and disputes in nursing should occur.
