[目的]探讨躯体感觉诱发电位(SEP)和脑电图(EEG)监测在颅内动脉瘤夹闭术中的应用价值。[方法]30例颅内动脉瘤在手术夹闭中持续进行SEP和EEG监测,对电生理信号异常者及时采取干预措施,并对其疗效进行分析。[结果]30例患者中11例术中电生理信号无异常改变,14例电生理信号异常,经干预后恢复正常,术后CT均未出现新的缺血性改变、神经功能良好。5例异常者经干预后未恢复正常,术后CT出现新的缺血性改变、并有新的神经功能缺失。[结论]动脉瘤夹闭术中持续监测SEP、EEG有助于及早发现和干预手术操作引起的脑缺血,对减少手术并发症有重要作用。
[目的]探討軀體感覺誘髮電位(SEP)和腦電圖(EEG)鑑測在顱內動脈瘤夾閉術中的應用價值。[方法]30例顱內動脈瘤在手術夾閉中持續進行SEP和EEG鑑測,對電生理信號異常者及時採取榦預措施,併對其療效進行分析。[結果]30例患者中11例術中電生理信號無異常改變,14例電生理信號異常,經榦預後恢複正常,術後CT均未齣現新的缺血性改變、神經功能良好。5例異常者經榦預後未恢複正常,術後CT齣現新的缺血性改變、併有新的神經功能缺失。[結論]動脈瘤夾閉術中持續鑑測SEP、EEG有助于及早髮現和榦預手術操作引起的腦缺血,對減少手術併髮癥有重要作用。
[목적]탐토구체감각유발전위(SEP)화뇌전도(EEG)감측재로내동맥류협폐술중적응용개치。[방법]30례로내동맥류재수술협폐중지속진행SEP화EEG감측,대전생리신호이상자급시채취간예조시,병대기료효진행분석。[결과]30례환자중11례술중전생리신호무이상개변,14례전생리신호이상,경간예후회복정상,술후CT균미출현신적결혈성개변、신경공능량호。5례이상자경간예후미회복정상,술후CT출현신적결혈성개변、병유신적신경공능결실。[결론]동맥류협폐술중지속감측SEP、EEG유조우급조발현화간예수술조작인기적뇌결혈,대감소수술병발증유중요작용。
[Objective] To explore the value of somatosensory evoked potentials (SEP) and electroencephalo-gram(EEG) monitoring used in the clipping of intracranial aneurysm .[Methods]SEP and EEG were continuously monitored in 30 patients undergoing the clipping of intracranial aneurysm on 30 patients .Intervention measures were taken timely for patients with abnormal electrophysiological signals .The efficacy was analyzed .[Results]Among 30 patients ,11 patients had no change of abnormal electrophysiological signals during the operation ,and 14 patients had abnormal electrophysiological signals ,and were recovered after intervention ,and showed no new ischemic changes by CT and no neurological deficits after the operation .Other 5 patients with abnormal electro-physiological signals were not recovered after intervention ,and showed new ischemic changes by CT ,and devel-oped functional deficits after the operation .[Conclusion]Continuous monitoring of SEP and EEG during the clip-ping of intracranial aneurysm is helpful for early finding and timely intervening cerebral ischemia caused by opera-tive procedure ,and plays an important role in reducing the surgical complications .
