目的:探讨CMSXIO4.64制定鼻咽癌调强放疗计划,子野权重优化时迭代次数的选择对计划结果的影响。方法选择10例鼻咽癌放疗病例,采用CMSXIO4.64制定调强放疗计划。在SWO 过程中,调整迭代次数形成不同计划,在不影响临床靶区剂量要求的前提下,比较其子野数,机器跳数及正常组织受照剂量。结果对同一个患者在一定范围内的选择迭代次数进行SWO,形成的不同计划之间相比较,总机器跳数和子野数变化不明显。同时脑干、脊髓和腮腺的受照剂量基本没变化。结论在使用CMSXIO4.64制定鼻咽癌的调强放疗计划过程中,适当的缩减迭代次数进行SWO,能在不增加机器跳数、子野数和危及器官受照剂量的条件下,缩短计划的优化时间,提高物理师的工作效率。

Objective To investigate the effect of iterative times on segment weight optimization ( SWO ) in intensity-modulated radiation therapy for nasopharyngeal carcinoma with CMSXIO 4.64.Methods 10 patients with nasopharyngeal carci-noma who received radiotherapy were selected .CMSXIO4.64 was used to formulate the intensity modulated radiation therapy ( IM-RT) .The iterative times were adjusted for formation of different plans in the process of SWO .Under the premise of not affecting the dose requirements within the clinical target volume ,segment number ,monitor unit and radiation doses were compared .Results The same patient who underwent selection of iterative times for SWO within a certain range .The total monitor unit and segment number were compared among different programs and were found to be not significantly changed .Meanwhile ,there were basically no changes in radiation doses to brainstem ,spinal cord and parotid gland .Conclusion In the process of IMRT for nasopharyngeal carcinoma with CMSXIO4.64,appropriate reduction of iterative times for SWO can reduce the optimization time and improve the work efficiency of the physics under the condition of not increasing monitor unit ,segment number and radiation doses to organs at risk.
