目的:探讨神经电生理检查对吉兰-巴雷综合征早期诊断的价值。方法:对入组的67例病人的神经电生理检查进行回顾性分析,其中病程5 d~4 w,最终诊断为吉兰-巴雷综合征的37例病人录入研究组(玉组),另外30例正常人录入对照组(域组)进行分析,两组分别进行上下肢神经传导速度、波幅及F波检测。上肢主要检查正中神经、尺神经的感觉运动传导速度、波幅及尺神经F波检测;下肢主要检查腓总神经、胫神经运动神经传导速度、波幅及腓肠神经传导速度、波幅及胫神经F波检测,对全部结果进行分析。结果:在受检神经中,玉组感觉神经、运动神经的传导速度、波幅及F波潜伏期、出现率异常较域组有显著差异(P<0.001)。结论:神经传导速度、波幅及F波潜伏期、出现率检测可以作为吉兰-巴雷综合征早期诊断的一个重要手段,在结合临床症状及体征的基础上,可以有效的提高对吉兰-巴雷综合征早期诊断率,从而到达早期治疗,缩短病程,改善病人临床转归的目的。
目的:探討神經電生理檢查對吉蘭-巴雷綜閤徵早期診斷的價值。方法:對入組的67例病人的神經電生理檢查進行迴顧性分析,其中病程5 d~4 w,最終診斷為吉蘭-巴雷綜閤徵的37例病人錄入研究組(玉組),另外30例正常人錄入對照組(域組)進行分析,兩組分彆進行上下肢神經傳導速度、波幅及F波檢測。上肢主要檢查正中神經、呎神經的感覺運動傳導速度、波幅及呎神經F波檢測;下肢主要檢查腓總神經、脛神經運動神經傳導速度、波幅及腓腸神經傳導速度、波幅及脛神經F波檢測,對全部結果進行分析。結果:在受檢神經中,玉組感覺神經、運動神經的傳導速度、波幅及F波潛伏期、齣現率異常較域組有顯著差異(P<0.001)。結論:神經傳導速度、波幅及F波潛伏期、齣現率檢測可以作為吉蘭-巴雷綜閤徵早期診斷的一箇重要手段,在結閤臨床癥狀及體徵的基礎上,可以有效的提高對吉蘭-巴雷綜閤徵早期診斷率,從而到達早期治療,縮短病程,改善病人臨床轉歸的目的。
목적:탐토신경전생리검사대길란-파뢰종합정조기진단적개치。방법:대입조적67례병인적신경전생리검사진행회고성분석,기중병정5 d~4 w,최종진단위길란-파뢰종합정적37례병인록입연구조(옥조),령외30례정상인록입대조조(역조)진행분석,량조분별진행상하지신경전도속도、파폭급F파검측。상지주요검사정중신경、척신경적감각운동전도속도、파폭급척신경F파검측;하지주요검사비총신경、경신경운동신경전도속도、파폭급비장신경전도속도、파폭급경신경F파검측,대전부결과진행분석。결과:재수검신경중,옥조감각신경、운동신경적전도속도、파폭급F파잠복기、출현솔이상교역조유현저차이(P<0.001)。결론:신경전도속도、파폭급F파잠복기、출현솔검측가이작위길란-파뢰종합정조기진단적일개중요수단,재결합림상증상급체정적기출상,가이유효적제고대길란-파뢰종합정조기진단솔,종이도체조기치료,축단병정,개선병인림상전귀적목적。
Objective:Discussing the early diagnosis value of electro-physiological tests in Guillan Barre Syndrome( GBS) . Methods:This study was carried out to analyze and compare 67 patients whose courses of disease were 5 days to 4 weeks. 37 clinically diagnosed GBS patients were put into the studying group( Group I) and the other 30 healthy human beings were put into the control group( Group II) . The nerve conduction velocities, amplitudes and F-waves of their upper and lower limbs were tested in both of the groups. Upper limbs:motor and sensation nerve conduction velocities,amplitudes of both median,ulnar nerves and F-wave of ulnar nerve were checked;lower limbs:conduction velocities and amplitudes of peroneal, tibial motor, sural nerves and F-wave of tibial nerve were tested and analyzed. Results:of all the tested persons,abnormalities of conduction velocities,amplitudes and F-wave latencies of sensory and motor nerves in Group I can be obviously seen compared with Group II( P<0. 001). Conclusion:conduction velocities of nerves, amplitudes, F-wave latencies and rate of emergence tests play an important role in early diagnosis of GBS. Combined with clinical symptoms and signs, they can help to improve the early diagnosis, early treatment of GBS patients, cut down their courses of disease.
