目的探讨1H-MRS技术在脑多发性硬化(multiple sclerosis,MS)的诊断及定性中的应用价值。材料与方法选择29例经临床确诊的MS患者进行MR及1H MRS检查,并与同期相同年龄组MRI显示正常者进行对照研究,计算1H-MRS代谢产物N-乙酰天冬氨酸(NAA)、肌酸(Cr)、胆碱复合物(Cho)及乳酸(LAC)的峰下面积,并对NAA/(Cr+Cho)、Cho/Cr及LAC/Cr等各参数进行对比研究。结果MS组NAA/(Cr+Cho)、Cho/Cr及LAC/Cr值分别为0.56、1.49及3.35,对照组分别为0.78、1.19及0.23。MS患者NAA/(Cr+Cho)值下降,Cho/Cr、LAC/Cr值升高,差异有统计学意义(P值均<0.05)。结论1H MRS有很高的特异性,可以为MS的早期诊断提供可靠的诊断信息,对指导临床治疗和判断预后有重要意义。

목적탐토1H-MRS기술재뇌다발성경화(multiple sclerosis,MS)적진단급정성중적응용개치。재료여방법선택29례경림상학진적MS환자진행MR급1H MRS검사,병여동기상동년령조MRI현시정상자진행대조연구,계산1H-MRS대사산물N-을선천동안산(NAA)、기산(Cr)、담감복합물(Cho)급유산(LAC)적봉하면적,병대NAA/(Cr+Cho)、Cho/Cr급LAC/Cr등각삼수진행대비연구。결과MS조NAA/(Cr+Cho)、Cho/Cr급LAC/Cr치분별위0.56、1.49급3.35,대조조분별위0.78、1.19급0.23。MS환자NAA/(Cr+Cho)치하강,Cho/Cr、LAC/Cr치승고,차이유통계학의의(P치균<0.05)。결론1H MRS유흔고적특이성,가이위MS적조기진단제공가고적진단신식,대지도림상치료화판단예후유중요의의。
Objective:To assess the utility of 1H magentic resonance spectroscopy (1H-MRS) in diagnosing and characterizing multiple sclerosis (MS). Materials and Methods: 1H-MRS was carried out in 29 patients with MS and 29 age-matched healhy control subjects. The areas under the peaks of the metabolites of N-acetylaspartic acid (NAA), creatine (Cr), choline compounds (Cho) and lactic acid (Lac) were calculated. The metabolics of NAA/(Cr+Cho), Cho/Cr and Lac/Cr were compared between the two groups. Results:The value of NAA/(Cr+Cho), Cho/Cr and LAC/Cr of MS group and control group were 0.56, 1.49, 3.35 and 0.78, 1.19, 0.23, respectively. Compared with the control group, NAA/(Cr+Cho) reduced, Cho/Cr and Lac/Cr increased in the MS group, all of which had staistically singniifcance (P<0.05). Conclusions: 1H-MRS has high speciifcity, can provide reliable diagnostic information for early diagnosis of MS, and may have important signiifcance to guide clinical treatment and judging prognosis.
