目的：分析四肢骨肉瘤患者的治疗方法及预后。方法回顾性分析182例四肢骨肉瘤患者的临床病理资料、治疗方法及生存情况。结果182例骨肉瘤患者位于肱骨16例（8．8％），桡骨1例（0．5％），股骨者109例（59．9％），胫骨48例（26．4％），腓骨8例（4．4％）；其中行保肢手术92例（50．5％），截肢手术90例（49．5％），规范化疗81例（44．5％），未规范化疗101例（55．5％）；182例患者随访时间为34～62个月，3年生存率为58．8％；单因素分析结果显示：年龄、肺部转移、手术方式、化疗是否规范及Enneking分期与3年生存率相关（P＜0．05）；COX回归分析结果显示：手术方式、化疗是否规范及肺转移是影响预后的独立相关因素。结论新辅助化疗联合保肢手术可延长四肢骨肉瘤患者生存期，充分切除肿瘤可改善患者预后。
목적：분석사지골육류환자적치료방법급예후。방법회고성분석182례사지골육류환자적림상병리자료、치료방법급생존정황。결과182례골육류환자위우굉골16례（8．8％），뇨골1례（0．5％），고골자109례（59．9％），경골48례（26．4％），비골8례（4．4％）；기중행보지수술92례（50．5％），절지수술90례（49．5％），규범화료81례（44．5％），미규범화료101례（55．5％）；182례환자수방시간위34～62개월，3년생존솔위58．8％；단인소분석결과현시：년령、폐부전이、수술방식、화료시부규범급Enneking분기여3년생존솔상관（P＜0．05）；COX회귀분석결과현시：수술방식、화료시부규범급폐전이시영향예후적독립상관인소。결론신보조화료연합보지수술가연장사지골육류환자생존기，충분절제종류가개선환자예후。
Objective To study the treatment and prognosis of osteosarcoma of extremities .Methods The clinical path-ological data ,treatment methods and survival situation of 182 patients with osteosarcoma of extremities were retrospectively ana-lyzed.Results Among the 182 patients,there were 16 cases of humerus (8.8%),1 case of radius(0.5%),109 cases of femur (59.9%),48 cases of tibia(26.4%) and 8 cases of fibula (4.4%);The treatment methods included 92(50.5%) cases of limb salvage surgery and 90(49.5%)cases of amputation surgery;81(44.5%)cases of standard chemotherapy and 101(55.5%)ca-ses without standard chemotherapy;The follow-up time was 34~62(42.30 ±2.58)months.The 3-year survival rate was 58.8%;Single factor analysis showed that age ,lung metastasis,operative methods,chemotherapy and Enneking surgical classification were related with 3-years survival rate ( P<0.05);COX regression analysis showed that operative methods ,chemotherapy and lung metastasis were related independent prognostic factors .Conclusion Neo-adjuvant chemotherapy combined with limb salvage sur-gery for osteosarcoma of extremities can prolong patients survival time ,and extended resection can improve prognosis of patients .
