目的:探讨儿童肾病综合征(NS)患者血清视黄醇结合蛋白4(RBP4)水平的变化及其与氧化低密度脂蛋白(ox-LDL)的关系。方法分别检测118例NS患儿和45名健康体检儿童(正常对照组)血清RBP4水平,同时测定其血清ox-LDL、白蛋白(Alb)、血脂[包括总胆固醇(TC)、甘油三酯(TG)、低密度脂蛋白胆固醇(LDL-C)、高密度脂蛋白胆固醇(HDL-C)及非高密度脂蛋白胆固醇(non HDL-C)]及肾功能指标[尿素(Urea)、肌酐(Cr)],并做统计学分析。结果 NS组血清RBP4水平为(55.88±27.36)mg/L,明显高于正常对照组[(30.61±8.68)mg/L](P<0.01),且血清RBP4与ox-LDL、TC、TG、LDL-C、non HDL-C及Urea呈明显正相关[相关系数(r)分别为0.30、0.35、0.25、0.29、0.32、0.26,P均<0.01];进一步多元回归分析显示,NS组TC(β=0.62,P<0.01)、Urea(β=0.26,P<0.01)和ox-LDL(β=0.20,P<0.05)水平共同影响了48.4%的RBP4的变化,且RBP4也是NS患儿ox-LDL水平的决定因素之一。结论 NS患儿血清RBP4水平明显升高。升高的RBP4与ox-LDL的增加有一定的关联,可能促进了NS的发生、发展。

목적:탐토인동신병종합정(NS)환자혈청시황순결합단백4(RBP4)수평적변화급기여양화저밀도지단백(ox-LDL)적관계。방법분별검측118례NS환인화45명건강체검인동(정상대조조)혈청RBP4수평,동시측정기혈청ox-LDL、백단백(Alb)、혈지[포괄총담고순(TC)、감유삼지(TG)、저밀도지단백담고순(LDL-C)、고밀도지단백담고순(HDL-C)급비고밀도지단백담고순(non HDL-C)]급신공능지표[뇨소(Urea)、기항(Cr)],병주통계학분석。결과 NS조혈청RBP4수평위(55.88±27.36)mg/L,명현고우정상대조조[(30.61±8.68)mg/L](P<0.01),차혈청RBP4여ox-LDL、TC、TG、LDL-C、non HDL-C급Urea정명현정상관[상관계수(r)분별위0.30、0.35、0.25、0.29、0.32、0.26,P균<0.01];진일보다원회귀분석현시,NS조TC(β=0.62,P<0.01)、Urea(β=0.26,P<0.01)화ox-LDL(β=0.20,P<0.05)수평공동영향료48.4%적RBP4적변화,차RBP4야시NS환인ox-LDL수평적결정인소지일。결론 NS환인혈청RBP4수평명현승고。승고적RBP4여ox-LDL적증가유일정적관련,가능촉진료NS적발생、발전。
Objective To investigate the relationship between the level change of serum retinol binding protein 4 (RBP4)and oxidized-low density lipoprotein(ox-LDL)in children with nephrotic syndrome(NS).Methods The levels of serum RBP4 in 1 1 8 children with NS (NS group ) and 45 healthy children (healthy control group ) were determined respectively,the levels of ox-LDL,albumin (Alb ),blood lipids [total cholesterol (TC ),triglyceride (TG),low-density lipoprotein cholesterol (LDL-C),high-density lipoprotein cholesterol (HDL-C)and non HDL-C] and renal function parameters [urea and creatinine (Cr)]were determined simultaneously,and the results were analyzed statistically.Results The levels of serum RBP4 in NS group [(55.88 ±27.36)mg/L]were significantly higher than those in healthy control group [(30.61 ±8.68)mg/L](P <0.01 ).The levels of serum RBP4 were positively correlated with ox-LDL,TC,TG,LDL-C,non HDL-C and urea [correlation coefficient (r)=0.30,0.35,0.25, 0.29,0.32 and 0.26,P<0.01 ].Multiple linear regression analysis found that the changes of TC(β=0.62,P<0.01 ),urea(β=0.26,P<0.01 )and ox-LDL(β=0.20,P<0.05)influenced the 48.4% of the change of RBP4.In addition,RBP4 was one of factors for the levels of ox-LDL in children with NS.Conclusions The levels of serum RBP4 in children with NS increase significantly,which are associated with the increase of ox-LDL,promoting the occurrence and development of NS.
