目的：探讨血清癌胚抗原（CEA）、神经元特异性烯醇化酶（NSE）、肾上腺髓质素（ADM ）对肺癌诊断及病理分型的意义。方法采用电化学发光法检测中国人民解放军第163医院193例初诊肺癌患者及47例肺部良性疾病患者的血清CEA、NSE、ADM 的水平。结果肺癌患者血清CEA、NSE、ADM 的平均水平均明显高于肺部良性疾病患者。在肺癌不同组织类型中CEA、NSE、ADM 的表达水平差异有统计学意义；CEA在肺腺癌中表达水平最高；NSE在肺小细胞癌中表达水平最高；ADM 在肺鳞癌中表达水平最高。结论血清CEA、NSE、ADM对肺癌的诊断及病理分型具有指导意义，三者联合检测可提高对肺癌诊断的敏感度，指导临床采用合理的治疗方案。

Objective To explore the value of conventional biochemical markers in clinical diagnosis of the pleural ffusion ,including glucose (GLU) ,adenosine deaminase (ADA) ,lactate ehydrogenase (LDH) ,of carcino-embryonic antigen (CEA) ,cytokeratin 19 fragment (CYRFA21-1) neuron-specific of enolase (NSE) .Methods 127 clinical cases of pleural effusion from July 2010 to July 2013 were review .The cases were divided into three groups:the TB group ,malignant tumor group and non-tuberculous benign group .To analyze the test results of GLU ,ADA ,LDH ,CEA ,CYRFA21-1and NSE .Results It was statistically significant of Glu ,LDH bbetween the TB group ,malignant tumor group and non-tuberculous benign group (P<0 .01 or P<0 .05);It was statisti-cally significant for ADA bbetween the tuberculous group and the other two groups (P<0 .01 or P<0 .05);It was statistically significant for CEA ,NSE ,CYFRA21 bbetween the malignant group and the other two groups (P<0 .01) .Conclusion Combined detection of pleural effusion and ascites for GLU ,ADA ,LDH ,NSE ,CEA and CYRFA21-1 has important reference value to differential diagnosis of pleural effusion .
