目的:探讨尿肺炎链球菌抗原检测在肺炎链球菌肺炎诊断中的临床意义。方法随机选择下呼吸道感染患儿300例,同时采集患儿痰标本、血标本、尿标本,对痰、血标本进行病原菌培养、鉴定,采用胶体金法检测尿肺炎链球菌抗原。结果300例患儿中,痰培养、血培养和尿肺炎链球菌抗原检测阳性率分别为10.33%(31/300)、15.67%(47/300)和19.33%(58/300),尿肺炎链球菌抗原检测阳性率高于其他两种方法,血培养检测阳性率高于痰培养(P<0.05);尿肺炎球链菌抗原检测诊断肺炎链球菌肺炎的灵敏度和特异度分别为82.98%(39/47)和92.49%(234/253)。结论尿肺炎链球菌抗原检测可作为儿童肺炎链球菌肺炎的辅助诊断依据。

목적:탐토뇨폐염련구균항원검측재폐염련구균폐염진단중적림상의의。방법수궤선택하호흡도감염환인300례,동시채집환인담표본、혈표본、뇨표본,대담、혈표본진행병원균배양、감정,채용효체금법검측뇨폐염련구균항원。결과300례환인중,담배양、혈배양화뇨폐염련구균항원검측양성솔분별위10.33%(31/300)、15.67%(47/300)화19.33%(58/300),뇨폐염련구균항원검측양성솔고우기타량충방법,혈배양검측양성솔고우담배양(P<0.05);뇨폐염구련균항원검측진단폐염련구균폐염적령민도화특이도분별위82.98%(39/47)화92.49%(234/253)。결론뇨폐염련구균항원검측가작위인동폐염련구균폐염적보조진단의거。
Objective To explore the clinical significance of urinary Streptococcus pneumoniae (Sp)antigen detection for the diagnosis of Sp pneumonia.Methods Sputum samples and blood samples,collected from 300 pediat-ric inpatients with lower respiratory tract infections,were detected for pathogenic bacteria by culture and identifica-tion,and urine samples,collected at the same time from the same inpatients,were detected for Sp antigen by colloidal gold method.Results Among 300 pediatric inpatients,the positive rates of sputum culture,blood culture and urinary Sp antigen test were 10.33%(31/300),15.67%(47/300)and 19.33%(58/300)respectively.The positive rate of u-rinary Sp antigen test was higher than the other two methods,and the positive rate of blood culture was higher than sputum culture (P<0.05).The sensitivity and specificity of the determination of urinary Sp antigen were 82.98%(39/47)and 92.49%(234/253).Conclusion Urinary Sp antigen detection could be used for assisted diagnosis of Sp pneumonia in children.
