目的：针对关节科高龄患者意外拔管较为高发的现状，探寻有效的护理干预措施。方法我科100例留置管道患者随机分为试验组和对照组各50例，试验组对肢体行保护性约束，对照组行常规护理，无保护性约束。对2组护理干预效果进行比较。结果试验组采用保护性约束100%防止了意外拔管，50例患者只有1例患者/家属配合程度较差；对照组不采取保护性约束，出现6例意外拔管，有2例患者/家属配合程度较差。结论对于高龄患者给予适当的肢体保护性约束，再辅助以良好的沟通及适当的镇静药物治疗，可有效防止意外拔管脱管。
목적：침대관절과고령환자의외발관교위고발적현상，탐심유효적호리간예조시。방법아과100례류치관도환자수궤분위시험조화대조조각50례，시험조대지체행보호성약속，대조조행상규호리，무보호성약속。대2조호리간예효과진행비교。결과시험조채용보호성약속100%방지료의외발관，50례환자지유1례환자/가속배합정도교차；대조조불채취보호성약속，출현6례의외발관，유2례환자/가속배합정도교차。결론대우고령환자급여괄당적지체보호성약속，재보조이량호적구통급괄당적진정약물치료，가유효방지의외발관탈관。
Objective Focused on higher frequency of unplanned extubation on elderly patients in our hospital joint surgery,find out the effective nursing interventions. Methods The investigation and analysis was carried out based on 100 cases of catheter tubing clinical data, the comparison was done to get the right nursing way between protective constraint and general nursing without protective constraint. Results The prevention and intervention effect is better for unplanned extubation with the nursing way of protective constraint, comparing with the actions without protective constraint. Conclusion The proper protective constraint, aligned with good communication and right sedative medication, is effective for prevention of unplanned extubation on elderly patients.
