Complex anal fistula%Fistulography%MSCT
目的:探讨使用多层螺旋 CT 瘘道造影三维重建技术对复杂性肛瘘进行诊断的临床应用价值。资料与方法:收集经手术证实的24例复杂性肛瘘患者,经皮下瘘口注入30%泛影葡胺水溶液后,行多层螺旋 CT 横断面扫描及计算机 SSD、VRT、 MPR 后处理,测量病灶长度与宽度,并明确其数目及走行方向。结果:以多层螺旋 CT 瘘道造影横断面图像信息为基础并结合 MPR 冠矢状位、SSD 、VRT 图像观察,能显示宽窄不一、走行方向不同、形态各异的瘘管及分支,并明确内口位置及其与周围组织的解剖关系。结论:利用多层螺旋 CT 的优良性能并结合瘘道造影,可直观展示复杂性肛瘘,从而为手术方案制定及术后观察提供科学依据。

목적:탐토사용다층라선 CT 루도조영삼유중건기술대복잡성항루진행진단적림상응용개치。자료여방법:수집경수술증실적24례복잡성항루환자,경피하루구주입30%범영포알수용액후,행다층라선 CT 횡단면소묘급계산궤 SSD、VRT、 MPR 후처리,측량병조장도여관도,병명학기수목급주행방향。결과:이다층라선 CT 루도조영횡단면도상신식위기출병결합 MPR 관시상위、SSD 、VRT 도상관찰,능현시관착불일、주행방향불동、형태각이적루관급분지,병명학내구위치급기여주위조직적해부관계。결론:이용다층라선 CT 적우량성능병결합루도조영,가직관전시복잡성항루,종이위수술방안제정급술후관찰제공과학의거。
Objective : To study the applied value of MSCT three dimensional reconstruction image combined with MSCT fistulography in complex anal fistula. Methods : To review 24 cases of complex anal fistula confirmed by operation. Al the patients were scanned after injected contrast medium through the fistula canales.then performed restruction by MPR (multi planar reformation) , SSD (shaded surface display) and VRT(volume rendering technology),measured length and width of lesions, identified sum and site of lesions.Result: On routine sectional view of MSCT fistulography combined with image of restruction, the shape and the branch of fistula can be displayed wel , the correlation between anal fistula and surrounding tissue can be revealed, the site of inner orificium can be showed accurately.Conclusion:MSCT combined with fistulography can display complex anal fistula directly , thus it is a reliable method for operative guidance.
