目的:系统评价预防性应用抗生素预防乳腺癌手术后手术部位感染的疗效。方法检索1975年至2013年3月公开发表的所有预防性使用抗菌药物对乳腺癌手术后手术部位感染影响的随机对照试验,进行荟萃分析及系统评价。结果筛选符合纳入标准的随机对照试验7项,共2220例患者;共有150例发生感染,感染率为6.76%。其中预防性应用抗生素组1115例,有53例发生感染,感染率为4.75%;对照组1105例,有97例发生感染,感染率为8.75%。分析发现,与对照组相比,预防性使用抗菌药物组可以减少手术部位感染48%( OR 0.52,95%CI 0.37~0.73,P=0.0002)。结论预防性应用抗生素能有效降低乳腺癌手术后手术部位感染率。

목적:계통평개예방성응용항생소예방유선암수술후수술부위감염적료효。방법검색1975년지2013년3월공개발표적소유예방성사용항균약물대유선암수술후수술부위감염영향적수궤대조시험,진행회췌분석급계통평개。결과사선부합납입표준적수궤대조시험7항,공2220례환자;공유150례발생감염,감염솔위6.76%。기중예방성응용항생소조1115례,유53례발생감염,감염솔위4.75%;대조조1105례,유97례발생감염,감염솔위8.75%。분석발현,여대조조상비,예방성사용항균약물조가이감소수술부위감염48%( OR 0.52,95%CI 0.37~0.73,P=0.0002)。결론예방성응용항생소능유효강저유선암수술후수술부위감염솔。
Objective We conducted a systematic review and meta-analysis of the public literature to evaluate the efficacy of prophylactic antibiotics in the prevention of surgical site infection after breast cancer surgery . Methods All the literature of the randomized controlled trials about the efficacy of prophylactic antibiotics in the prevention of surgical site infection after breast cancer surgery published from January 1975 to March 2013 was retrivaled and systematically reviewed .Results A total of 7 randomized controlled trials meeting the inclusion criteria were screened.Among the 2220 cases of patients,150 cases had wound infection ,and the infection rate was 6.76%.In the prophylactic antibiotics group of 1115 cases,wound infection occurred in 53 cases;the infection rate was 4.75%.In the control group of 1105 cases,97 cases had incision infection;the infection rate was 8.75%.The analysis found that , compared with the control group , the preventive use of antimicrobial drugs group can reduce surgical site infection after breast cancer surgery for 48%( OR 0.52 , 95% CI 0.37-0.73 , P =0.0002 ) . Conclusion The use of prophylactic antibiotics can effectively reduce the surgical site infection after breast cancer surgery.
