目的:评价珊瑚羟基磷灰石人工骨用于良性骨肿瘤切除后重建的效果,并与异体松质骨作比较。方法整理从2004年12月至2008年7月我院使用珊瑚羟基磷灰石与异体松质骨植骨的两组病例各62例。两组病例在治疗时间、年龄、性别、病种、病灶部位、植骨量和随访时间等方面均匹配良好。通过影像学和临床随访,评估并比较植骨的愈合速度和愈合率,分析影响人工骨愈合的因素。结果羟基磷灰石的初步愈合率为62.9%,异体骨的初步愈合率为96.8%,差异有统计学意义( P=0.000);羟基磷灰石的平均初步愈合时间为(13.3±7.5)个月,异体骨平均初步愈合时间为(8.8±4.9)个月,差异有统计学意义(P =0.000)。性别( P=0.369)、年龄( P=0.290)、病灶部位( P=0.704)、病种( P=0.994)和植骨量( P=0.348)均不是影响羟基磷灰石愈合情况的显著因素。羟基磷灰石组与异体骨组患者术后MSTS平均分分别为(28.8±1.8)分和(29.0±1.5)分,两组无显著差异(P=0.450)。结论羟基磷灰石用于良性骨肿瘤切除后骨缺损的重建,术后功能满意,但愈合率和愈合速度均低于异体骨。

목적:평개산호간기린회석인공골용우량성골종류절제후중건적효과,병여이체송질골작비교。방법정리종2004년12월지2008년7월아원사용산호간기린회석여이체송질골식골적량조병례각62례。량조병례재치료시간、년령、성별、병충、병조부위、식골량화수방시간등방면균필배량호。통과영상학화림상수방,평고병비교식골적유합속도화유합솔,분석영향인공골유합적인소。결과간기린회석적초보유합솔위62.9%,이체골적초보유합솔위96.8%,차이유통계학의의( P=0.000);간기린회석적평균초보유합시간위(13.3±7.5)개월,이체골평균초보유합시간위(8.8±4.9)개월,차이유통계학의의(P =0.000)。성별( P=0.369)、년령( P=0.290)、병조부위( P=0.704)、병충( P=0.994)화식골량( P=0.348)균불시영향간기린회석유합정황적현저인소。간기린회석조여이체골조환자술후MSTS평균분분별위(28.8±1.8)분화(29.0±1.5)분,량조무현저차이(P=0.450)。결론간기린회석용우량성골종류절제후골결손적중건,술후공능만의,단유합솔화유합속도균저우이체골。
Objective To observe and compare the effect of coral hydroxyapatite artificial bone and allograft in reconstruction of bone defect after benign bone tumor resection .Methods Sixty cases received artificial bone and 60 cases received allogenic cancellous bone grafts from December 2004 to July 2008 in our center were studied .Two groups matched well in age ,gender,disease,site of lesion,bone graft volume and the follow-up time.Through imaging and clinical follow-up, we compared bone graft healing speed and healing rate between two groups .The influence factors on artificial bone healing were analyzed .Results The initial healing rates of artificial bone and allograft were 62.9%and 96.8%, respectively ( P =0.000 );the mean initial healing time of artificial bone and allograft were (13.3 ±7.5)months and(8.8 ±4.9)months,respectively(P=0.000).Gender(P=0.369),age(P=0.290),site ( P=0.704 ) , disease ( P =0.994 ) and bone graft volume ( P =0.348 ) were not significant factors affecting bone healing.The mean MSTS scores of artificial bone and allograft group were 28.8 ±1.8 and 29.0 ±1.5 ,respectively ( P=0.450 ) .Conclusion The patients with benign limb bone tumor have good functional recovery after coral hydroxyapatite artificial bone implantation .Compared to allograft ,hydroxyapatite artificial bone show low healing rate and healing speed .
