目的：探讨早期帕金森病(PD)患者抑郁状况与健康相关生活质量(health-related quality of life, HR-QOL)的关系。方法筛选出87例早期PD患者入组。采用统一帕金森病评分表(UPDRS)和Hoehn-Yahr评价运动症状，采用流行病学研究中心编制的抑郁量表(CES-D)对抑郁症状进行评价，采用36条目简化医疗结局调查问卷(SF-36)评价HR-QOL。探讨早期帕金森病(PD)患者抑郁状况与健康相关生活质量(health-related quality of life, HR-QOL)的关系。结果87例早期PD患者中有51例存在抑郁状态（CES-D≥16分），无抑郁状态患者36例（对照组）。抑郁状态组患者除躯体疼痛(BP)维度外，其余各维度分值较对照组均下降。躯体功能(PF)；躯体问题导致的角色受限(RP)；总体健康(GH)；活力(VT)；社交功能(SF)；情感问题导致的角色受限(RE)；精神健康(MH)等维度评分均明显低于对照组（P≤0.05）。结论抑郁症状严重影响早期PD患者的HR-QOL，是导致早期PD患者HR-QOL恶化的主要因素。
목적：탐토조기파금삼병(PD)환자억욱상황여건강상관생활질량(health-related quality of life, HR-QOL)적관계。방법사선출87례조기PD환자입조。채용통일파금삼병평분표(UPDRS)화Hoehn-Yahr평개운동증상，채용류행병학연구중심편제적억욱량표(CES-D)대억욱증상진행평개，채용36조목간화의료결국조사문권(SF-36)평개HR-QOL。탐토조기파금삼병(PD)환자억욱상황여건강상관생활질량(health-related quality of life, HR-QOL)적관계。결과87례조기PD환자중유51례존재억욱상태（CES-D≥16분），무억욱상태환자36례（대조조）。억욱상태조환자제구체동통(BP)유도외，기여각유도분치교대조조균하강。구체공능(PF)；구체문제도치적각색수한(RP)；총체건강(GH)；활력(VT)；사교공능(SF)；정감문제도치적각색수한(RE)；정신건강(MH)등유도평분균명현저우대조조（P≤0.05）。결론억욱증상엄중영향조기PD환자적HR-QOL，시도치조기PD환자HR-QOL악화적주요인소。
Objective To investigate the relationship between depression and health-related quality of life in patients with early Parkinson’s disease.Methods All 87 patients with early PD were identified in a clinical-based study.Motor functions were measured by Uniifed Parkinson’s Disease Rating Scale (UPDRS) and Hoehn-Yahr Scale. Depressive symptoms were assessed by Center of Epidemiological Survey Depression Scale (CES-D). HR-QOL was measured by SF-36. To study the relationship between depression and health-related quality of life in patients with early Parkinson’s disease.Results In all 87 patients with early PD ,51 patients were in the depression state(CES-D≥16 score), and 36 patients were not in the depression state(control group). Except bodily pain dimension,patients with early PD in the depression state had a lower score on all dimensions of SF-36,including physical function, role limitation-physical,general health, vitality, social function, role limitation-emotional and metal health. Compared with control group, the score decreased respectively with signiifcance (P≤0.05).Conclusions Depressive symptoms has a substantial impact on HR-QOL in patients with early Parkinson’s disease.Depressive symptoms correlated strongly with lower quality of life.
