目的：探讨全髋关节置换术治疗股骨头缺血性坏死的临床疗效。方法对32例股骨头缺血性坏死ARCO分期Ⅲ、Ⅳ期的患者应用全髋关节置换手术治疗。结果32例患者术后疼痛的减轻及关节功能改善明显，生活质量得到极大提高，所有患者均获得长期随访，一例患者术后三年出现髋臼假体松动现象。结论全髋关节置换术具有减轻疼痛，改善关节功能，显著提高生活质量，是治疗50岁以上股骨头缺血性坏死ARCO分期Ⅲ、Ⅳ期患者的一种有效的治疗方法。
목적：탐토전관관절치환술치료고골두결혈성배사적림상료효。방법대32례고골두결혈성배사ARCO분기Ⅲ、Ⅳ기적환자응용전관관절치환수술치료。결과32례환자술후동통적감경급관절공능개선명현，생활질량득도겁대제고，소유환자균획득장기수방，일례환자술후삼년출현관구가체송동현상。결론전관관절치환술구유감경동통，개선관절공능，현저제고생활질량，시치료50세이상고골두결혈성배사ARCO분기Ⅲ、Ⅳ기환자적일충유효적치료방법。
Objective To explore the clinical effect of total hip replacement in the treatment of ischemic necrosis of femoral head. Methods patients with total hip arthroplasty in the treatment of 32 cases of avascular necrosis of the femoral head in ARCO stageⅢ,Ⅳstage. Results of the 32 patients with postoperative pain relief and functional improvement, quality of life greatly improved, all patients were available for long-term follow-up, found no loosening of prosthesis. Conclusion total hip arthroplasty can relieve pain, improve joint function, signiifcantly improve the quality of life, is over 50 years old with avascular necrosis of the femoral head in ARCO stage III, IV patients an effective treatment method.
