目的:了解尿路感染大肠埃希菌产超广谱β-内酰胺酶(ESBLs)株的耐药情况和主要基因型。方法纸片扩散法(K-B)检测细菌对抗菌药物的敏感性;用表型确证试验确定临床尿液标本中产ESBLs的大肠埃希菌。应用聚合酶链反应(PCR)扩增菌株的CTX-M、TEM 和SHV型ESBLs基因,并对 PCR产物经凝胶电泳后进行初步分析。结果大部分产酶菌株对美罗培南、亚胺培南、头孢西丁等耐药性较低,而对氨苄青霉素、哌拉西林和头孢噻肟等耐药性较高。在72株 ESBLs阳性菌株中,CTX-M 型所占比例为48.6%,TEM 型占87.5%,SHV 型占0.0%,同时含有CTX-M和TEM两种基因型的占47.2%。结论尿路感染大肠埃希菌产ESBLs株的耐药情况较为严重,CTX-M和TEM是其主要基因型。应加强监控,防止产酶菌株的播散。

목적:료해뇨로감염대장애희균산초엄보β-내선알매(ESBLs)주적내약정황화주요기인형。방법지편확산법(K-B)검측세균대항균약물적민감성;용표형학증시험학정림상뇨액표본중산ESBLs적대장애희균。응용취합매련반응(PCR)확증균주적CTX-M、TEM 화SHV형ESBLs기인,병대 PCR산물경응효전영후진행초보분석。결과대부분산매균주대미라배남、아알배남、두포서정등내약성교저,이대안변청매소、고랍서림화두포새우등내약성교고。재72주 ESBLs양성균주중,CTX-M 형소점비례위48.6%,TEM 형점87.5%,SHV 형점0.0%,동시함유CTX-M화TEM량충기인형적점47.2%。결론뇨로감염대장애희균산ESBLs주적내약정황교위엄중,CTX-M화TEM시기주요기인형。응가강감공,방지산매균주적파산。
Objective To investigate the antibiotic resistance situation and genotypes of extended-spectrum β-Lactamases(ESBLs)-producing Escherichia coli(E .coli) in urinary tract infection (UTIs) .Methods The antibiotic susceptibility test was performed by Kirby-Bauer method .The ESBLs-producing E .coli was confirmed by phenotypic confirmatory test .The genotypes of ESBLs were examined by polymerase chain reaction (PCR) and nucleic acid elec-trophoresis .Results The ESBLs-producing E .coli were more sensitive to meropenem ,imipenem and cefoxitin ,but resistant to ampicillin ,piperacillin and cefotaxime .48 .6% of the ESBLs positive isolates was found carried CTX-M gene ,87 .5% of the ESBLs positive isolates was found carried TEM gene ,and no isolate was found carried SHV gene .47 .2% of the strains were found carried both the CTX-M and TEM genes .Conclusion The antibiotic resist-ance situation of the ESBLs-producing E .coli in UTIs could be very serious .CTX-M and TEM genes might be the predominant genetypes of the ESBLs-producing E .coli in UTIs .
