目的：探讨尿激酶胸腔注射在结核性胸膜炎中的临床应用价值。方法：选取2010年1月～2013年1月本院收治的78例初治结核性胸膜炎，实验组40例采用生理盐水20ml+尿激酶12.5WU胸腔注入治疗，对照组采用生理盐水20ml胸腔注入治疗，10d后判断疗效及临床重要指标评估。结果：两组患者的临床疗效进行比较分析发现实验组显效率和总有效率均明显高于对照组；各临床指标比较结果发现实验组胸水减少速度、胸膜肥厚、平均住院时间及胸膜粘连发生率均优于对照组。结论：尿激酶胸腔注射治疗结核性胸膜炎，可以提高疗效、缩短疗程、降低胸膜粘连发生率，可以作为治疗胸腔积液的重要辅助手段，具有很重要的临床使用价值。

목적：탐토뇨격매흉강주사재결핵성흉막염중적림상응용개치。방법：선취2010년1월～2013년1월본원수치적78례초치결핵성흉막염，실험조40례채용생리염수20ml+뇨격매12.5WU흉강주입치료，대조조채용생리염수20ml흉강주입치료，10d후판단료효급림상중요지표평고。결과：량조환자적림상료효진행비교분석발현실험조현효솔화총유효솔균명현고우대조조；각림상지표비교결과발현실험조흉수감소속도、흉막비후、평균주원시간급흉막점련발생솔균우우대조조。결론：뇨격매흉강주사치료결핵성흉막염，가이제고료효、축단료정、강저흉막점련발생솔，가이작위치료흉강적액적중요보조수단，구유흔중요적림상사용개치。
Objective:To investigate the value of clinical application of urokinase intrapleural injection in the diagnosis of tuberculous pleurisy. Methods:from 2010 January to 2013 January in our hospital 78 cases of initial treatment of tuberculous pleurisy, 38 cases in experimental group using saline 20ml+urokinase intrapleural injection of 12.5WU treatment, the control group using saline 20ml pleural injection treatment, judging the curative effect and clinical index evaluation after 10 days. Results: the clinical efficacy of the two groups were comparative analysis found that the experimental group markedly effective rate and total effective rate were significantly higher than those in control group; the clinical indexes of comparative experimental results show that the group of pleural effusion, pleural thickening and reduce speed, average hospitalization time and the incidence rate of pleural adhesion was bet er than the control group. Conclusion: urokinase intrapleural injection in treatment of tuberculous pleurisy, can improve the efficacy, shorten the course of treatment, reduce the incidence rate of pleural adhesion, can be used as an important adjunct to the treatment of pleural effusion, has very important clinical value in use.
