目的研究脓毒症新生儿心肌损害( MI)时外周血sCD14和sVCAM-1的变化及意义。方法选取脓毒症新生儿50例,其中MI 20例为脓毒症MI组,其余30例为脓毒症组;同期正常新生儿20例为对照组。所有研究对象均留取1~1.5ml抗凝全血分离血浆,采用酶联免疫双抗体夹心(ABC-ELISA)法检测血浆sCD14和sVCAM-1的浓度。结果 sCD14和sVCAM-1的表达水平在新生儿脓毒症MI组均最高,与脓毒症组和对照组的比较,差异有显著性,两者在新生儿脓毒症组的表达水平也显著高于对照组( F=43.538,P<0.05;F=170.237,P<0.05)。结论 CD14和VCAM-1参与了新生儿脓毒症MI的炎症损害过程,动态观察,有助于早期发现和预测脓毒症新生儿MI的发生。

목적연구농독증신생인심기손해( MI)시외주혈sCD14화sVCAM-1적변화급의의。방법선취농독증신생인50례,기중MI 20례위농독증MI조,기여30례위농독증조;동기정상신생인20례위대조조。소유연구대상균류취1~1.5ml항응전혈분리혈장,채용매련면역쌍항체협심(ABC-ELISA)법검측혈장sCD14화sVCAM-1적농도。결과 sCD14화sVCAM-1적표체수평재신생인농독증MI조균최고,여농독증조화대조조적비교,차이유현저성,량자재신생인농독증조적표체수평야현저고우대조조( F=43.538,P<0.05;F=170.237,P<0.05)。결론 CD14화VCAM-1삼여료신생인농독증MI적염증손해과정,동태관찰,유조우조기발현화예측농독증신생인MI적발생。
Objective To investigate Changes and Significences of the peripheral blood sCD14 and sVCAM-1 in the Myocardial injury of Neonatal sepsis. Method sCD14 and sVCAM-1 levels were measured by using double-antibody sandwich enzyme-linked immunosorbent ( ABC-ELISA) method in 20 cases with MI in neonatal sepsis, 30 cases with mild neonatal sepsis and 20 healthy neonates. Result The plasma sCD14 and sVCAM-1 levels in the sepsis-MI group were all the highest among the three groups, statistically different with those in the other two groups, and the plasma levels in the mild neonatal sepsis group were also higher than those in the control group( F=43. 538,P<0. 05;F=170. 237,P<0. 05). Conclusion CD14 and VCAM-1 may be involved in the pathogenesis of neonatal sepsis and its MI. Dynamic observation of the changes of the peripheral blood sCD14 and sVCAM-1 levels could be used to diagnosis early and evaluate the prognosis of neonatal sepsis and MI.
