目的改良阴式子宫切除术治疗非脱垂良性病变大子宫的临床应用效果。方法回顾性分析非脱垂子宫大于孕12周的良性病变患者98例,68例行改良阴式子宫切除术(改良组),同期30例行经腹子宫切除术(开腹组),对两组临床资料进行回顾性分析。结果改良组手术成功率100%,无1例发生副损伤或转为开腹手术。改良组手术时间、出血量、排气时间、下床活动时间、住院时间及术后病率均明显少于开腹组,差异有显著性(P<0.05)。两组子宫平均重量比较,差异无显著性(P>0.05)。结论治疗非脱垂良性病变大子宫,与经腹子宫切除术相比,如果掌握手术适应证和手术技巧,改良的阴式子宫切除术具有手术时间短,术中出血少,术后恢复快等优势,是一种安全、有效的子宫切除术式。
목적개량음식자궁절제술치료비탈수량성병변대자궁적림상응용효과。방법회고성분석비탈수자궁대우잉12주적량성병변환자98례,68례행개량음식자궁절제술(개량조),동기30례행경복자궁절제술(개복조),대량조림상자료진행회고성분석。결과개량조수술성공솔100%,무1례발생부손상혹전위개복수술。개량조수술시간、출혈량、배기시간、하상활동시간、주원시간급술후병솔균명현소우개복조,차이유현저성(P<0.05)。량조자궁평균중량비교,차이무현저성(P>0.05)。결론치료비탈수량성병변대자궁,여경복자궁절제술상비,여과장악수술괄응증화수술기교,개량적음식자궁절제술구유수술시간단,술중출혈소,술후회복쾌등우세,시일충안전、유효적자궁절제술식。
Objective To explore the clinical efficacy of improved TVH . Method A retro spective analysis was made on clinical date of 68 cases improved TVH( Vaginal Group) and 30 cases of abdominal total hysterectomy( Ab-dominal Group) . The patient perioperative condition in two groups was compared and analyzed. Result There were statistically significant differences on the operation time,intraoperative bleeding,period of exhaust,out-of-bed activi-ty,average hospital days and postoperative morbidity between the Vaginal Group and the Abdominal Group ( P<0. 05). There were no statistically significant differences on sizes of uterus between two groups(P>0. 05). Conclu-sion Improved transvaginal hysterectomy is superior to abdominal hysterectomy in big non-prolapse uterus,Improved transvaginal hysterectomy has little damage, quick recovery and shorter length of hospital stay, few complications. It is worth to be generalized in clinic.
