目的:通过检测色素上皮衍生因子(PEDF)在2型糖尿病肾病不同时期患者血清中的表达水平,从而研究 PEDF 在糖尿病肾病(DN)发生发展中的作用,为早期诊断糖尿病肾病提供更敏感的临床检测指标。方法选择研究对象共150例,分为健康对照组、2型糖尿病组,糖尿病肾病Ⅲ期、Ⅳ期、Ⅴ期组,每组30例,分别检测空腹血清PEDF,静脉血糖(FPG),糖化血红蛋白(HbA1c)、三酰甘油(TG)、总胆固醇(TC)、血肌酐(sCr)。结果健康对照组、糖尿病组、糖尿病肾病Ⅲ期组、Ⅳ期组、Ⅴ期组患者血清中 PEDF 水平依次逐渐增高,且各组间比较差异有统计学意义( P <0.05)。 Person 相关性分析显示 PEDF 与 FPG、HbA1C、TC、TG、LDL 呈正相关( r 值分别为0.510、0.56、0.462、0.397、0.411, P <0.01),与 HDL 呈负相关( r =-0.325, P <0.01)。结论2型糖尿病肾病患者血清 PEDF 水平显著升高,并且随着病情进展逐渐增加,血清 PEDF 水平升高可能在 DN 的发生发展过程中起一定代偿保护作用,血清PEDF 水平可能成为早期诊断 DN 的一个敏感指标。

목적:통과검측색소상피연생인자(PEDF)재2형당뇨병신병불동시기환자혈청중적표체수평,종이연구 PEDF 재당뇨병신병(DN)발생발전중적작용,위조기진단당뇨병신병제공경민감적림상검측지표。방법선택연구대상공150례,분위건강대조조、2형당뇨병조,당뇨병신병Ⅲ기、Ⅳ기、Ⅴ기조,매조30례,분별검측공복혈청PEDF,정맥혈당(FPG),당화혈홍단백(HbA1c)、삼선감유(TG)、총담고순(TC)、혈기항(sCr)。결과건강대조조、당뇨병조、당뇨병신병Ⅲ기조、Ⅳ기조、Ⅴ기조환자혈청중 PEDF 수평의차축점증고,차각조간비교차이유통계학의의( P <0.05)。 Person 상관성분석현시 PEDF 여 FPG、HbA1C、TC、TG、LDL 정정상관( r 치분별위0.510、0.56、0.462、0.397、0.411, P <0.01),여 HDL 정부상관( r =-0.325, P <0.01)。결론2형당뇨병신병환자혈청 PEDF 수평현저승고,병차수착병정진전축점증가,혈청 PEDF 수평승고가능재 DN 적발생발전과정중기일정대상보호작용,혈청PEDF 수평가능성위조기진단 DN 적일개민감지표。
Objective To investigate the role of pigment epithelium derived factor (PEDF) in the pathogenesis and development of diabetic nephropathy (DN) through detecting the expression levels of PEDF in serum in patients with type 2 diabetes mellitus at different stages ,in order to provide more sensitive clinical detection indexes for early diagnosis of DN . Methods The 150 subjects were divided into five groups ,health control group,type 2 diabetes mellitus group,DN Ⅲ stage group,DN Ⅳ stage group and DN Ⅴ stage group,with 30 cases in each group.The fasting serum PEDF,vein FPG,glycosylated hemoglobin (HbA1c),TG,TC,sCr were detected by ELISA.Results PEDF levels in health control group,type 2 diabetes mellitus group,DN Ⅲ stage group,DN Ⅳ stage group and DN Ⅴ stage group were in turn increased,and there were significant differences among the five groups ( P <0.05).Person correlation analysis showed that PEDF was positively correlated with FPG,TC,TG,HbA1c,LDL,HDL ( r =0.510,0.56,0.462,0.397,0.411,respectively, P <0.01),however,which was negatively correlated to HDL ( r =-0.325, P <0.01).Conclusion The serum PEDF levels are obviously increased in patients with DN,furthermore,which increase gradually with the development of pathogenetic condition .The increase of serum PEDF levels may play certain compensation protection effect during the pathogenesis and development of DN ,which may become a sensitive index in early diagnosis of DN .
