目的:研究细胞角蛋白19(CK19)、半乳糖凝集素-3(Gal-3)和间皮瘤相关抗体-1(HBME-1)在甲状腺良恶性结节中的表达及其意义。方法采用免疫组织化学方法检测75例甲状腺良恶性结节中 CK19、Gal-3和HBME-1蛋白表达,良性结节36例(甲状腺滤泡性腺瘤14例,结节性甲状腺肿22例),恶性结节39例(甲状腺乳头状癌36例,甲状腺滤泡癌3例)。结果 CK19、Gal-3和 HBME-1蛋白表达在良性结节的阳性表达率分别为41.7%、25.0%、11.1%,在恶性结节的阳性表达率均为94.9%;CK19、Gal-3和 HBME-1在诊断恶性结节的敏感性均为94.9%,特异性分别为33.3%、69.4%、88.9%。结论3项指标中,HBME-1在诊断恶性结节的敏感性和特异性最为理想;CK19、Gal-3和 HBME-1三者联合使用在甲状腺良恶性结节诊断和鉴别诊断中具有实用价值。

목적:연구세포각단백19(CK19)、반유당응집소-3(Gal-3)화간피류상관항체-1(HBME-1)재갑상선량악성결절중적표체급기의의。방법채용면역조직화학방법검측75례갑상선량악성결절중 CK19、Gal-3화HBME-1단백표체,량성결절36례(갑상선려포성선류14례,결절성갑상선종22례),악성결절39례(갑상선유두상암36례,갑상선려포암3례)。결과 CK19、Gal-3화 HBME-1단백표체재량성결절적양성표체솔분별위41.7%、25.0%、11.1%,재악성결절적양성표체솔균위94.9%;CK19、Gal-3화 HBME-1재진단악성결절적민감성균위94.9%,특이성분별위33.3%、69.4%、88.9%。결론3항지표중,HBME-1재진단악성결절적민감성화특이성최위이상;CK19、Gal-3화 HBME-1삼자연합사용재갑상선량악성결절진단화감별진단중구유실용개치。
Objective To investigate the expression and significance of CK19,Gal-3 and HBME-1 in benign and malignant thyroid nodules.Methods The expression of CK19,Gal-3 and HBME-1 in 75 cases of be-nign and malignant thyroid nodulesprotein was detected by Immunohistochemistry (36 cases of the benign nodules including 14 cases of follicular adenoma and 22 cases of nodular goiter,39 cases of the malignant nodules including 36 cases of thyroid papillary carcinoma and 3 cases of follicular thyroid cancer).Results The positive rate of CK19,Gal-3 and HBME-1 in thyroid benign nodules were 41.7%,25.0%,11.1% re-spectively,and in thyroid malignant nodules was 94.9%.The sensitivity rate of CK19,Gal-3 and HBME-1 in thyroid malignant nodules were 94.9%,and the rate of specificity were 33.3%、69.4%、88.9% respec-tively.Conclusion HBME-1 is the most ideal index of three kinds of index in terms of sensitivity and spe-cificity in the diagnosis of malignant nodules.The joint use of CK19,Gal-3 and HBME-1 is of practical val-ue in the diagnosis and identification of benign and malignant thyroid nodules.
