目的：探讨类风湿膝关节炎高频彩色多普勒超声表现与实验室检查的相关性。方法：对40例类风湿患者58个病变膝关节与30例正常人30个正常膝关节行高频彩色多普勒超声检查。观察内容：髌上囊积液、滑膜及其内血供、股骨下端关节软骨及骨皮质变化、有无腘窝囊肿形成等。分析滑膜厚度及髌上囊积液深度与血沉及C反应蛋白的相关性。结果：类风湿患者膝关节出现不同程度滑膜增厚，髌上囊积液增多。采用Spearman相关分析，髌上囊积液深度与血沉相关系数0.306，P＜0.05，与C反应蛋白相关系数0.293，P＜0.05；滑膜厚度与ESR及CRP均未见明确相关性。结论：高频超声可以清晰显示类风湿关节炎膝关节病变，其与实验室检查有一定相关性，可在一定程度上为诊断类风湿提供依据。
목적：탐토류풍습슬관절염고빈채색다보륵초성표현여실험실검사적상관성。방법：대40례류풍습환자58개병변슬관절여30례정상인30개정상슬관절행고빈채색다보륵초성검사。관찰내용：빈상낭적액、활막급기내혈공、고골하단관절연골급골피질변화、유무객와낭종형성등。분석활막후도급빈상낭적액심도여혈침급C반응단백적상관성。결과：류풍습환자슬관절출현불동정도활막증후，빈상낭적액증다。채용Spearman상관분석，빈상낭적액심도여혈침상관계수0.306，P＜0.05，여C반응단백상관계수0.293，P＜0.05；활막후도여ESR급CRP균미견명학상관성。결론：고빈초성가이청석현시류풍습관절염슬관절병변，기여실험실검사유일정상관성，가재일정정도상위진단류풍습제공의거。
Objective:To explore the correlation between manifestations of high frequency color Doppler ultrasound and laboratory examination of knee joints lesions in patients with rheumatoid arthritis(RA).Methods:40 patients of 58 knee joints with rheumatoid arthritis and 30 health adults of 30 knee joints were checked.We observed suprapatellar pouch effusion,synovial membrane and its blood supply,distal femoral articular cartilage and cortical bone changes,with or without popliteal cyst and so on. We analyzed the correlation among the synovial thickness,suprapatellar pouch effusion,erythrocyte sedimantation rate(ESR) and C-reactive protein(CRP).Results:There were different degrees of synovial thickness and suprapatellar pouch effusion in knee joints of patients with RA.By using Spearman analysis,the correlation coefficient was 0.306(P<0.05) between the degree of suprapatellar pouch effusion and erythrocyte sedimantation rate,the correlation coefficient was 0.293(P<0.05) between the degree of suprapatellar pouch effusion and C-reactive protein.Conclusion:The high frequency ultrasound can clearly show the knee joints disease of rheumatoid arthritis,there is a certain relationship between it and laboratory examination,and it can provide the basis for the diagnosis of rheumatoid arthritis in a certain extent.
