[目的]探讨不同剂量地塞米松对芬太尼所致咳嗽反射(FIC )的抑制作用及对血糖的影响。[方法]选取全麻下行择期手术患者300例,随机分为三组,每组100例。在静注芬太尼前5 min ,分别推注等量生理盐水(A组)、地塞米松5 mg (B组)、地塞米松10 mg (C组),观察并比较各组FIC的发生率、严重程度、发生时间及不同时点的血糖值。[结果]B、C组FIC发生率显著低于A组( P<0.05);三组FIC严重程度、发生时间比较无显著性差异( P >0.05);C组在静注后60 min、120 min、180 min血糖值高于 A、B组( P <0.05)。[结论]预先静注5 mg地塞米松可有效的抑制FIC ,且对血糖影响较小。

[목적]탐토불동제량지새미송대분태니소치해수반사(FIC )적억제작용급대혈당적영향。[방법]선취전마하행택기수술환자300례,수궤분위삼조,매조100례。재정주분태니전5 min ,분별추주등량생리염수(A조)、지새미송5 mg (B조)、지새미송10 mg (C조),관찰병비교각조FIC적발생솔、엄중정도、발생시간급불동시점적혈당치。[결과]B、C조FIC발생솔현저저우A조( P<0.05);삼조FIC엄중정도、발생시간비교무현저성차이( P >0.05);C조재정주후60 min、120 min、180 min혈당치고우 A、B조( P <0.05)。[결론]예선정주5 mg지새미송가유효적억제FIC ,차대혈당영향교소。
[Objective] To explore the efficacy of different dose of dexamethasone for reducing the inci-dence of fentanyl-induced cough(FIC) and its influence on blood glucose .[Methods] Totally 300 patients un-dergoing elective surgical procedures were chosen and randomly divided into 3 groups with 100 patients in each group .Each group was injected with isovolume normal saline (group A) ,dexamethasone 5mg(group B) and dexamethasone 10mg(group C) respectively at 5min before injection of fentanyl .The incidence ,severity ,oc-currence time of FIC and blood glucose value at each time point in all groups were observed and compared .[Results] The incidence of FIC in group B and group C was significantly lower than that in group A ( P <0 .05) .There was no significant difference in the severity and occurrence time of FIC among 3 groups( P >0 .05) .Blood glucose value in group C at 60min ,120min and 180min after intravenous injection was higher than that in group A and group B( P<0 .05) .[Conclusion] Pre-injection of dexamethasone 5mg can effective-ly inhibit FIC ,and has less influence on blood glucose .
