目的:探讨不同靶浓度瑞芬太尼应用于老年患者胸外科手术麻醉的价值。方法收集2011年1月~2013年12月,本院胸外科住院并接受手术治疗的患者93例,随机分为A、B、C三组,各31例,三组麻醉诱导时分别应用瑞芬太尼靶浓度分别为4μg/L、6μg/L、8μg/L,比较三组各时间段的MAP、SBP及HR、手术情况以及去甲肾上腺素及血浆肾上腺素。结果 A组在T2、T4以及T5时的血压水平及HR水平均显著升高,而B组和C组的心率及血压升高抑制效果好, C组T1时MAP、SBP及HR均显著降低;A组在T2时EN及E水平均显著升高, B、C组显著低于A组;T3时三组的HR、血压、NE以及E水平均较T0时显著升高。结论在老年胸外科手术麻醉中应用6μg/L瑞芬太尼联合异丙酚靶控输注能够有效减轻患手术应激反应,确保手术安全,值得推广应用。

목적:탐토불동파농도서분태니응용우노년환자흉외과수술마취적개치。방법수집2011년1월~2013년12월,본원흉외과주원병접수수술치료적환자93례,수궤분위A、B、C삼조,각31례,삼조마취유도시분별응용서분태니파농도분별위4μg/L、6μg/L、8μg/L,비교삼조각시간단적MAP、SBP급HR、수술정황이급거갑신상선소급혈장신상선소。결과 A조재T2、T4이급T5시적혈압수평급HR수평균현저승고,이B조화C조적심솔급혈압승고억제효과호, C조T1시MAP、SBP급HR균현저강저;A조재T2시EN급E수평균현저승고, B、C조현저저우A조;T3시삼조적HR、혈압、NE이급E수평균교T0시현저승고。결론재노년흉외과수술마취중응용6μg/L서분태니연합이병분파공수주능구유효감경환수술응격반응,학보수술안전,치득추엄응용。
Objective To investigate the value of different target concentrations of remifentanil in elderly patients with thoracic surgery anesthesia.Methods 93 patients undergoing thoracic surgery in our hospital between January 2011 and December 2013 were collected.The patients were randomly divided into A, B, C three groups, 31 cases of each group, when the three groups were applied induction of anesthesia remifentanil concentrations were 4μg/L, 6μg/L, 8μg/L.The MAP, SBP and HR, the operation of plasma norepinephrine and epinephrine of the 3 groups were compared.Results At T2, T4 and T5 , the blood pressure levels and HR of group A were significantly increased, while in group B and group C, the heart rate and blood pressure is good inhibitory effect, T1 when the MAP, SBP and HR were significantly lower in group C ; A group were significantly higher at T2 and E EN water, B, C group was significantly lower than that in group A; T3 when the three groups of HR, BP, NE and E levels were significantly higher than those at T0.Conclusion 6μg/L remifentanil propofol in elderly anesthesia with thoracic surgery can effectively reduce the risk of surgical stress response, ensure safe operation, should be widely applied.
