目的：通过对强直性脊柱炎（AS）患者血清中白介素23（IL-23）水平的检测，研究其与AS疾病活动的相关性。方法用酶联免疫吸附法（ ELISA）检测60例AS患者和28例健康对照组的血清IL-23水平，分别比较患者组与健康对照组，病情活动组与稳定组，HLA-B27阳性组与阴性组及骶髂关节X线不同分期的IL-23水平，检测AS患者疾病活动性、临床指标（ CRP，ESR）、骨破坏程度，探讨其与IL-23水平的相关性。结果 AS组血清IL-23浓度高于健康对照组，差异有显著性（P＜0．05），AS患者IL-23的水平活动组高于稳定组，差异具有显著性（P＜0．05），AS患者HLA-B27阳性组与阴性组IL-23，CRP，ESR比较差异无显著性（P均＞0．05），AS患者血清IL-23水平与C反应蛋白、ESR呈正相关性（r值分别为0．333，0．433；P值均＜0．01），IL-23水平与骶髂关节X线破坏程度有关，差异具有显著性（ P＜0．05）。结论 AS患者血清IL-23水平比健康人升高，与疾病活动性相关，与骨破坏的程度相关，血清IL-23水平可作为AS活动性指标，可反映骨破坏程度。
목적：통과대강직성척주염（AS）환자혈청중백개소23（IL-23）수평적검측，연구기여AS질병활동적상관성。방법용매련면역흡부법（ ELISA）검측60례AS환자화28례건강대조조적혈청IL-23수평，분별비교환자조여건강대조조，병정활동조여은정조，HLA-B27양성조여음성조급저가관절X선불동분기적IL-23수평，검측AS환자질병활동성、림상지표（ CRP，ESR）、골파배정도，탐토기여IL-23수평적상관성。결과 AS조혈청IL-23농도고우건강대조조，차이유현저성（P＜0．05），AS환자IL-23적수평활동조고우은정조，차이구유현저성（P＜0．05），AS환자HLA-B27양성조여음성조IL-23，CRP，ESR비교차이무현저성（P균＞0．05），AS환자혈청IL-23수평여C반응단백、ESR정정상관성（r치분별위0．333，0．433；P치균＜0．01），IL-23수평여저가관절X선파배정도유관，차이구유현저성（ P＜0．05）。결론 AS환자혈청IL-23수평비건강인승고，여질병활동성상관，여골파배적정도상관，혈청IL-23수평가작위AS활동성지표，가반영골파배정도。
Objective Through the detection of serum Interleukin-23(IL-23)levels of ankylosing spondylitis(AS) patients,to study the correlation between it and AS activity.Methods The levels of serum IL-23 were detected in60 patients with AS and 28 cases from healthy control group with enzyme -linked immunosorbet assay(ELISA).Comparisonswas made among the AS group and healthy control group ,disease activity group and stable group,HLA-B27 positiveand negative group and X-ray of sacroiliac joint in different stages .The correlations of IL-23 with AS disease activity,index(CRP,ESR),bone destruction were detected.Results The serum IL-23 concentration of AS group was higher thanthat of healthy control group,and the difference was statistically significant.The levels of IL-23 in active AS patientsgroup were higher than those in the stable group,the difference was statistically significant.Among AS patients withHLA-B27 positive group and negative group,the levels of IL-23,CRP and ESR had no statistical significance (P >0.05).The serum IL-23 levels of AS patients were positively correlated with CRP ,ESR(r =0.333,0.433;P <0.01).Thedifference was statistically significant between the extent of damage and the level of IL -23 X-ray sacroiliac joint(P <0.05).Conclusion The levels of Serum IL-23 in AS patients are associated with bone destruction and disease activities .The levels of serum IL-23 can be used as AS activity index and can reflect the degree of bone destruction.
