目的：系统评价5型磷酸二酯酶抑制剂对治疗早泄患者及性功能正常人的临床疗效和安全性。方法检索国内外有关比较5型磷酸二酯酶抑制剂和安慰剂用于治疗早泄患者及性功能正常人的随机对照试验和随机交叉实验，由两名评价员独立按Cochrane系统评价方法对文献进行质量评价和提取资料后，采用RevMan 5.2统计软件进行系统评价，对于不能合并的效应量采用描述性研究。结果共纳入10个随机对照试验和随机交叉实验，512例患者。结果显示，5型磷酸二酯酶抑制剂可以提高早泄患者的射精潜伏期（IELT），缩短患者的射精后勃起潜伏期（PERT），提高性生活满意率。对于性功能正常人使用5型磷酸二酯酶抑制剂类药物可以缩短PERT，但能否提高IELT还存在一定争议。5型磷酸二酯酶抑制剂组副作用发生率较安慰剂组高。结论对于早泄患者可以尝试使用5型磷酸二酯酶抑制剂来治疗，对于性功能正常人要慎用5型磷酸二酯酶抑制剂。由于文献纳入的较少，需要更多大样本的随机对照试验进一步证实。

Objective To compare the efficacy and safety of phosphodiesterase type 5 inhibitors (PDE5-Is) in the treatment of premature ejaculation and normal sexual function through a systematic review.Methods All randomized controlled trials and randomized crossover trials of PDE5-Is and placebo in the treatment of premature ejaculation and normal sexual function were collected. The quality of included trials was assessed. Relevant data were selected according to the standard of Cochrane systematic review by two reviewers. Statistical software Revman 5.2 was used for systematic review.Results Ten randomized controlled trials and randomized crossover trials with 512 cases were included. For patients with the premature ejaculation, the PDE5-Is can prolong the intravaginal ejaculation latency time, decrease the post-ejaculatory refractory time and improve thesexual satisfaction rate. For people with normal sexual function, PDE5-Is caused a reduction of the post-ejaculatory refractory time. However, there was controversy about its effcet on the intravaginal ejaculation latency time. PDE5-Is have more adverse reaction.Conculsion For patients with the premature ejaculation, PDE5-Is was recommended to use in clinic, but for the normal sexual function people, PDE5-Is should be used cautionly. Because of the limited references,, more high quality randomized controlled trials with large samples are required to get further confirmation.
