目的：研究椎管内麻醉正中穿刺与侧入穿刺患者术后腰痛发生情况及其与应激状态的相关性。方法：选择椎管内麻醉的患者138例，随机分为进行侧入穿刺椎管内麻醉的观察组和进行正中穿刺椎管内麻醉的对照组，观察麻醉后腰痛发生例数、疼痛程度及应激状态。结果：观察组发生腰痛的例数、中国整脊颈腰痛评分、JOA下腰痛评分以及血糖水平、HAMD评分、HAMA评分和NRS评分均明显低于对照组，胰岛素水平明显高于对照组；血糖水平、HAMD评分、HAMA评分、NRS评分与中国整脊颈腰痛评分、JOA评分呈正相关关系；血胰岛素水平与整脊颈腰痛评分、JOA评分呈负相关关系。结论：侧入穿刺入路椎管内麻醉能够有效减少腰痛发生率、缓解腰痛程度和应激状态，具有积极的临床价值。
목적：연구추관내마취정중천자여측입천자환자술후요통발생정황급기여응격상태적상관성。방법：선택추관내마취적환자138례，수궤분위진행측입천자추관내마취적관찰조화진행정중천자추관내마취적대조조，관찰마취후요통발생례수、동통정도급응격상태。결과：관찰조발생요통적례수、중국정척경요통평분、JOA하요통평분이급혈당수평、HAMD평분、HAMA평분화NRS평분균명현저우대조조，이도소수평명현고우대조조；혈당수평、HAMD평분、HAMA평분、NRS평분여중국정척경요통평분、JOA평분정정상관관계；혈이도소수평여정척경요통평분、JOA평분정부상관관계。결론：측입천자입로추관내마취능구유효감소요통발생솔、완해요통정도화응격상태，구유적겁적림상개치。
Objective: To study the low back pain in patients received median and lateral puncture intravertebral anesthesia and its correlation with stress state. Methods:One hundred and thirty eight patients received intravertebral anesthesia were collected and randomly divided into observation group given lateral puncture intravertebral anesthesia and control group given median puncture intravertebral anesthesia. Then cases of low back pain, degree of low back pain and stress state were observed. Results:the cases of low back pain, Chinese chiropractic neck pain score, JOA score and blood glucose level, HAMD score, HAMA score, NRS score of observation group were significantly lower than control group, insulin levels was significantly higher control group;blood glucose level, HAMD score, HAMA score, NRS score were positively correlated with chiropractic neck pain score and JOA score; insulin levels was negatively correlated with chiropractic neck pain score and JOA score. Conclusion: lateral puncture intravertebral anesthesia has positive clinical value for it can effectively reduce the incidence of low back pain and relieve the degree of low back pain and stress state.
