目的:探讨CT灌注成像对肝细胞型肝癌(HCC)与局灶性结节增生(FNH)的鉴别诊断价值。方法比较14例HCC(A组)、10例FNH(B组)的实性成分的CT灌注值;比较FNH实性成分与远处肝组织CT灌注值;比较HCC实性成分、灶周组织及远处肝组织的CT灌注值。结果 A组的实性部分肝动脉灌注量(hepatic arterial perfusion,HAP)、总肝灌注量(total liver perfusion,TLP)、肝动脉灌注指数(hepatic artery perfusion index, HPI)都低于B组,门静脉灌注量(portal venous perfusion,PVP)高于B组,其中HAP、PVP及HPI存在统计学差异。A组病灶实性部分的HAP、HPI高于灶周、远处肝组织,并存在统计学意义;病灶实性部分的PVP低于灶周、远处肝组织,存在统计学差异。B组中病灶实性部分的HAP、HPI、TLP高于远处肝组织,PVP低于远处肝组织,其中HAP、HPI及PVP存在统计学差异。结论 CT灌注成像对肝癌及局灶性结节增生的诊断及鉴别诊断具有重要临床意义。
目的:探討CT灌註成像對肝細胞型肝癌(HCC)與跼竈性結節增生(FNH)的鑒彆診斷價值。方法比較14例HCC(A組)、10例FNH(B組)的實性成分的CT灌註值;比較FNH實性成分與遠處肝組織CT灌註值;比較HCC實性成分、竈週組織及遠處肝組織的CT灌註值。結果 A組的實性部分肝動脈灌註量(hepatic arterial perfusion,HAP)、總肝灌註量(total liver perfusion,TLP)、肝動脈灌註指數(hepatic artery perfusion index, HPI)都低于B組,門靜脈灌註量(portal venous perfusion,PVP)高于B組,其中HAP、PVP及HPI存在統計學差異。A組病竈實性部分的HAP、HPI高于竈週、遠處肝組織,併存在統計學意義;病竈實性部分的PVP低于竈週、遠處肝組織,存在統計學差異。B組中病竈實性部分的HAP、HPI、TLP高于遠處肝組織,PVP低于遠處肝組織,其中HAP、HPI及PVP存在統計學差異。結論 CT灌註成像對肝癌及跼竈性結節增生的診斷及鑒彆診斷具有重要臨床意義。
목적:탐토CT관주성상대간세포형간암(HCC)여국조성결절증생(FNH)적감별진단개치。방법비교14례HCC(A조)、10례FNH(B조)적실성성분적CT관주치;비교FNH실성성분여원처간조직CT관주치;비교HCC실성성분、조주조직급원처간조직적CT관주치。결과 A조적실성부분간동맥관주량(hepatic arterial perfusion,HAP)、총간관주량(total liver perfusion,TLP)、간동맥관주지수(hepatic artery perfusion index, HPI)도저우B조,문정맥관주량(portal venous perfusion,PVP)고우B조,기중HAP、PVP급HPI존재통계학차이。A조병조실성부분적HAP、HPI고우조주、원처간조직,병존재통계학의의;병조실성부분적PVP저우조주、원처간조직,존재통계학차이。B조중병조실성부분적HAP、HPI、TLP고우원처간조직,PVP저우원처간조직,기중HAP、HPI급PVP존재통계학차이。결론 CT관주성상대간암급국조성결절증생적진단급감별진단구유중요림상의의。
Objective To investigate the differential diagnosis value of CT perfusion imaging in hepatocellular carcinoma(HCC) and focal nodular hyperplasia(FNH). Methods Comparison of 14 cases of HCC (group A) and 10 cases of FNH (group B) CT perfusion value of solid part; Comparison of FNH of solid composition and the distant liver tissue CT perfusion value; Comparison of HCC solid component, Lesions surrounding tissue and distant liver tissue CT perfusion value. Results The group A solid part of the hepatic artery perfusion (HAP), the total amount of liver perfusion (TLP), hepatic artery perfusion index (HPI) is lower than that of group B, portal venous perfusion (PVP) higher than that of group B, the HAP, PVP and HPI statistical differences. Group A solid lesions component of HAP, HPI above focal weeks, liver tissue in the distance, and there is statistical significance; Solid lesions component of PVP below focal weeks, distant liver tissue, statistical differences. Solid lesions in group B component of HAP, the HPI, TLP is higher than the distant liver tissue, PVP is lower than the distant liver tissue, HAP, HPI and PVP statistical differences among them. Conclusion CT perfusion imaging of HCC and FNH of the diagnosis and differential diagnosis has important clinical significance.
