目的:在神经外科手术中观察对比不同剂量盐酸戊乙奎醚麻醉前黏膜给药对肌松、镇静、血流动力学及术中和术后腺体分泌的影响。方法:选择45例择期神经外科手术患者(ASAⅠ~Ⅱ级),采用随机分组的方式,将其分为三组,C组通过口鼻腔黏膜于麻醉前30 min注射用水1 mL,P1组给予盐酸戊乙奎醚0.5 mg,P2组给予盐酸戊乙奎醚1 mg。采用阻抗法通过在双盲的条件下观察给药前、后不同时段血流动力学改变,从而量化判断每组患者的口干程度,同时注重观察不同手术时段其对腺体分泌的影响。结果:盐酸戊乙奎醚在麻醉前黏膜给予对抑制腺体分泌影响明显,对心搏量、心输出量、心率无明显影响(P>0.05),盐酸戊乙奎醚可延长肌松剂的维持时间。结论:盐酸戊乙奎醚与抑制腺体分泌在麻醉前黏膜给予条件下呈剂量相关性,其对心血管无明显兴奋作用,可能延长肌松药的作用时间。

목적:재신경외과수술중관찰대비불동제량염산무을규미마취전점막급약대기송、진정、혈류동역학급술중화술후선체분비적영향。방법:선택45례택기신경외과수술환자(ASAⅠ~Ⅱ급),채용수궤분조적방식,장기분위삼조,C조통과구비강점막우마취전30 min주사용수1 mL,P1조급여염산무을규미0.5 mg,P2조급여염산무을규미1 mg。채용조항법통과재쌍맹적조건하관찰급약전、후불동시단혈류동역학개변,종이양화판단매조환자적구간정도,동시주중관찰불동수술시단기대선체분비적영향。결과:염산무을규미재마취전점막급여대억제선체분비영향명현,대심박량、심수출량、심솔무명현영향(P>0.05),염산무을규미가연장기송제적유지시간。결론:염산무을규미여억제선체분비재마취전점막급여조건하정제량상관성,기대심혈관무명현흥강작용,가능연장기송약적작용시간。
Objective:To observe the effects of Penehyclidine Hydrochloride giving from glue in premedicate in the neurosurgery surgical operation.Method:In a double-blind,randomized study,45 adult patients scheduled to neurosurgery surgical operation were assigned to three groups.P1 group(enehyclidine hydrochloride 0.5 mg plus injective water 0.5 mg),P2 group(enehyclidine hydrochloride 1 mg),C group(injective water 1 mg). The heart rate,invasive artery blood pressure,SV,CI,and gland secreting,muscle relaxation were recorded.Result:Penehyclidine hydrochloride giving from glue as premeditation could effect the gland secretion(P<0.01). Penehyclidine hydrochloride had significant stability on HR,SV and CI.Penehyclidine hydrochloride could prolong the time which T4/T1 recovered 25%.Conclusion:Penehyclidine hydrochloride,a new anticholinergic drug,has a significant stability on HR,has not a significant effection on SV,CI.Penehyclidine hydrochloride can decrease the muscle relaxant.
