目的：探讨加压螺钉固定于股骨头置换术治疗老年股骨颈骨折的疗效。方法31例患者接受加压螺钉内固定，31例实施人工股骨头置换，比较两组病例在手术时间、术中出血及术后引流量，髋关节功能Harris评分及并发症方面的差异。结果前者在手术时间、术中出血量及术后引流量方面优于后者，但Harris评分不如后者，差异有统计学意义（P<0.05）；但在术后并发症方面没有明显差异（P>0.05）。结论股骨颈骨折要根据骨折具体情况选用内固定方式或股骨头置换术，加强术后护理及康复锻炼，均可起到很好的效果。
목적：탐토가압라정고정우고골두치환술치료노년고골경골절적료효。방법31례환자접수가압라정내고정，31례실시인공고골두치환，비교량조병례재수술시간、술중출혈급술후인류량，관관절공능Harris평분급병발증방면적차이。결과전자재수술시간、술중출혈량급술후인류량방면우우후자，단Harris평분불여후자，차이유통계학의의（P<0.05）；단재술후병발증방면몰유명현차이（P>0.05）。결론고골경골절요근거골절구체정황선용내고정방식혹고골두치환술，가강술후호리급강복단련，균가기도흔호적효과。
Objective To investigate the compression screw fixation in femoral arthroplasty for femoral neck fractures. Methods 31 patients were treated with compression screw ifxation, 31 cases of implementation of hemiarthroplasty compared two groups of patients in the operative time, blood loss and postoperative drainage, Harris hip score and complications differences. The former results in operative time, blood loss and postoperative drainage is better than the latter, but Harris score than the latter, the difference was statistically signiifcant (P<0.05);but there were no signiifcant differences in postoperative complications (P>0.05). Conclusion fracture of femoral neck fracture according to speciifc circumstances or internal ifxation of femoral head replacement surgery, and postoperative care and rehabilitation exercises, can play a very good effect.
