目的 采用荟萃分析的方法评价组织工程皮肤(TES)预防腮腺切除术后并发症的有效性和安全性.方法 系统检索相关中英文数据库,包括CENTRAL(the Cochrane Central Register of Controlled Trials)、PubMed、EMBASE、中国期刊全文数据库(CNKI)、中国生物医学文献数据库(CBM)、中文科技期刊全文数据库(VIP)、万方数字化期刊全文数据库(Wanfang)等,筛选出符合要求的25个随机对照试验,评价方法学质量并提取数据,对味觉出汗综合征(GSS)等腮腺切除术后并发症进行荟萃分析.结果 荟萃分析结果显示,与术后直接缝合相比,应用TES可以使GSS客观发生率降低84%[ RR=0.16,95%CI(0.13 ~ 0.21),P<0.01]、GSS主观发生率降低86% [RR=0.14,95%C/(0.10 ~ 0.19),P<0.01];可以有效降低涎漏的发生[RR=0.23,95%CI(0.10 ~ 0.53),P<0.01],有效改善面部凹陷畸形[RR=0.42,95%CI(0.18~0.95),P=0.04];有降低暂时性面神经麻痹发生率和增加血清积液发生率的趋势,但差异无统计学意义(均P>0.05).结论 现有证据表明,使用TES可以有效预防腮腺切除术后GSS、面部凹陷畸形及涎漏的发生.

Objective To assess the efficacy and safety of tissue engineered skin (TES) for prevention of post-operative complications of parotidectomy by conducting a meta-analysis.Methods A total of 25 eligible randomized controlled trials were systematically searched via retrieval in Chinese and English relevant databases,including The Cochrane Central Register of Controlled Trials (CENTRAL),PubMed,EMBASE,China National Knowledge Infrastructure (CNKI),China Biomedical Literature Database (CBM),VIP Database for Chinese Technical Periodicals and Wanfang Databse.This was followed by quality appraisal and data extraction for conducting meta-analysis on the post-operative complications of parotidectomy as a result of gustatory sweating syndrome (GSS).Results As shown by the results of meta-analysis,TES resulted in a 84% reduction in objective incidence of GSS [ RR=0.16,95%CI(0.13-0.21 ),P<0.01 ] and a 86% decline in subjective incidence of GSS [RR=0.14,95% CI (0.10-0.19),P<0.01 ] as compared with those treated with suture only.In addition,TES was associated with reduced incidence of salivary leakage [RR=0.23,95%CI(0.10-0.53),P<0.01 ] and improved facial depressed deformity [RR=0.42,95% CI (0.18- 0.95),P=0.04].Furthermore,TES might be associated with reduced incidence of temporal facial nerve paralysis and increased incidence of serum retention (both P>0.05).Conclusion Current evidences suggest that TES is effective for prevention of post-parotidectomy GSS,facial depressed deformity and salivary leakage.
