目的:观察多奈哌齐联合胞磷胆碱钠对轻中度阿尔茨海默病的认知功能的影响。方法40例轻中度阿尔茨海默病分为多奈哌齐组和多奈哌齐联合胞磷胆碱钠组各20例,治疗组给予多奈哌齐联合胞磷胆碱钠,对照组给予多奈哌齐,2组均给予控制血压、血脂等常规治疗,连续服用16周。治疗前及治疗后16周分别采用简易智力状态量表(MMSE)、日常生活活动能力量表(ADL)、经颅多普勒(TCD)、副反应量表(TESS)评估患者。结果治疗前2组MMSE评分、ADL评分、RI值及TESS值比较均无显著性差异(P>0.05),治疗组治疗后与治疗前比较,ADL评分减少,差异具有统计学意义(P<0.05);治疗组MMSE亚项中记忆力、注意力和计算力、回忆力、语言能力评分与治疗前比较增加,对照组记忆力、注意力和计算力、回忆力评分与治疗前比较增加,差异具有统计学意义(P<0.05);治疗组患者所有检测血管治疗后与治疗前比较RI值减小,血管阻力指数降低,差异具有统计学意义(P<0.05);对照组患者仅LMCA治疗后与治疗前比较RI减小,差异具有统计学意义(P<0.05);TESS评分治疗前后比较差异无统计学意义(P>0.05)。结论多奈哌齐联合养胞磷胆碱钠可以改善轻中度阿尔茨海默病患者的日常生活活动能力、记忆力、注意力和计算力、回忆力及语言能力,增加颅脑血流量,安全性良好。

목적:관찰다내고제연합포린담감납대경중도아이자해묵병적인지공능적영향。방법40례경중도아이자해묵병분위다내고제조화다내고제연합포린담감납조각20례,치료조급여다내고제연합포린담감납,대조조급여다내고제,2조균급여공제혈압、혈지등상규치료,련속복용16주。치료전급치료후16주분별채용간역지력상태량표(MMSE)、일상생활활동능역량표(ADL)、경로다보륵(TCD)、부반응량표(TESS)평고환자。결과치료전2조MMSE평분、ADL평분、RI치급TESS치비교균무현저성차이(P>0.05),치료조치료후여치료전비교,ADL평분감소,차이구유통계학의의(P<0.05);치료조MMSE아항중기억력、주의력화계산력、회억력、어언능력평분여치료전비교증가,대조조기억력、주의력화계산력、회억력평분여치료전비교증가,차이구유통계학의의(P<0.05);치료조환자소유검측혈관치료후여치료전비교RI치감소,혈관조력지수강저,차이구유통계학의의(P<0.05);대조조환자부LMCA치료후여치료전비교RI감소,차이구유통계학의의(P<0.05);TESS평분치료전후비교차이무통계학의의(P>0.05)。결론다내고제연합양포린담감납가이개선경중도아이자해묵병환자적일상생활활동능력、기억력、주의력화계산력、회억력급어언능력,증가로뇌혈류량,안전성량호。
Objective To observe the effect of donepezil combined with citicoline sodium on cognitive function of mild to moderate Alzheimer's disease. Methods Forty patients with mild to moderate Alzheimer's disease were divided into donepezil group and donepezil combined with citicoline sodium group ,respectively ,20 cases each group.The patients in the treatment group were treated with donepezil combined with citicoline sodium ,the control group were given donepezil.Two groups were given conventional treatment to control blood pressure ,blood lipid ,and two groups were administered for 16 weeks.The mini mental state scale (MMSE) ,activities of daily living scale (ADL) ,transcranial Doppler(TCD) and treatment emergent symptom scale (TESS) were estimated before treatment and after 16 weeks'treatment. Results There were no significant differences in MMSE score ,ADL score ,RI value and TESS value between the two groups before treatment (P>0.05).The patients'ADL scores decreased in the treatment group compared with before treatment ,and the difference was significant (P< 0.05).The subitem in MMSE ,such as memory ,attention and computation ability ,recall ,language ability scores in the treatment group increased compared with those before treatment ;but in the control group ,only memory ,attention and computation ability ,re-call scores increased compared with before treatment ,and the difference was significant(P<0.05).The vascular resistance in-dex (RI) in all the detected vessels of the treatment group decreased compared with that before treatment ,and the difference was significant(P<0.05).The LMCA RI value decreased compared with that before treatment in the control group ,with a sig-nificant difference(P<0.05). Patients of two groups before and after treatment showed no significant difference in TESS score (P> 0.05).Conclusion Donepezil combined with sodium can improve the activities of daily living ,memory ,attention and computation ability ,recall and language capacity in the patients with mild to moderate Alzheimer's disease ,and increase cere-bral blood flow ,and with good security.
