目的:探讨人乳头瘤病毒感染(HPV)及负荷量对宫颈癌前病变及宫颈癌的作用。方法选择2009年3月-2012年3月在新疆医科大学附属肿瘤医院经病理检查确诊的宫颈病变患者1326例为研究对象,其中慢性宫颈炎1000例,CIN Ⅰ52例,CIN Ⅱ40例,CIN Ⅲ142例,宫颈癌92例,采用杂交捕获二代(HC‐Ⅱ)方法定量检测宫颈 HPV‐DNA 的含量。结果(1)慢性宫颈炎组、CIN 组、宫颈癌组 HPV‐DNA 阳性率分别为13.5%、67.9%、94.5%,各组阳性率随着宫颈病变的程度加重而增加(P <0.001)。(2)CIN Ⅰ组、CIN Ⅱ组、CNI Ⅲ组中HPV‐DNA 阳性率分别为46.2%、57.5%、78.8%,各组阳性率随着 CIN 级别增高而增高(P <0.001)。(3)在HPV‐DNA 负荷量>500时,HPV‐DNA 病毒负荷量随着宫颈病变程度的加重而增加,组间比较差异有统计学意义(P <0.001)。结论 HPV‐DNA 病毒感染及其负荷量与宫颈癌前病变及宫颈癌发生有关;HC‐Ⅱ检测对预测CIN 、宫颈癌发展有重要意义。

목적:탐토인유두류병독감염(HPV)급부하량대궁경암전병변급궁경암적작용。방법선택2009년3월-2012년3월재신강의과대학부속종류의원경병리검사학진적궁경병변환자1326례위연구대상,기중만성궁경염1000례,CIN Ⅰ52례,CIN Ⅱ40례,CIN Ⅲ142례,궁경암92례,채용잡교포획이대(HC‐Ⅱ)방법정량검측궁경 HPV‐DNA 적함량。결과(1)만성궁경염조、CIN 조、궁경암조 HPV‐DNA 양성솔분별위13.5%、67.9%、94.5%,각조양성솔수착궁경병변적정도가중이증가(P <0.001)。(2)CIN Ⅰ조、CIN Ⅱ조、CNI Ⅲ조중HPV‐DNA 양성솔분별위46.2%、57.5%、78.8%,각조양성솔수착 CIN 급별증고이증고(P <0.001)。(3)재HPV‐DNA 부하량>500시,HPV‐DNA 병독부하량수착궁경병변정도적가중이증가,조간비교차이유통계학의의(P <0.001)。결론 HPV‐DNA 병독감염급기부하량여궁경암전병변급궁경암발생유관;HC‐Ⅱ검측대예측CIN 、궁경암발전유중요의의。
Objective To explore HPV infection and loads on cervical carcinoma and Cervical Intraepithelial neoplasia .Methods 1 326 cervical cancer sufferers were confirmed by pathological examina‐tion from march 2009 to march 2012 and were taken as subjects ,including 1 000 chronic cervicitis cases , 52 CIN Ⅰ cases ,40 CIN cases ,142 CIN Ⅲ cases ,92 cervical cancer cases .Results (1) The rate of HR‐HPV infection was 13 .5% in cervicitis ,67 .9% in CIN ,94 .5% in Cervical Cancer ,each positive rate in‐creased as the degree of cervical lesion increases (P < 0 .001) ;(2) The rate of HR‐HPV infection was 46 .2% in CIN Ⅰ ,57 .5% in CIN Ⅱ ,78 .8% in CIN Ⅲ ,each positive rate increased as the CIN level increased (P < 0 .001 ) ; (3 ) The comparison among four groups show that the distribution of HPV quantity was significantly different if HPV‐DNA > 500 (P < 0 .001) .Conclusion The viral loads of HPV DNA have a correlation with the grade of Cervial Cancer ,HC‐ Ⅱ test has important significance in CIN and the cervical cancer screening .
