目的：探讨富集自体骨髓干细胞移植联合手术治疗骨折的效果及对骨折术后延迟愈合或不愈合的疗效。方法骨折患者60例，按随机数字表法分为治疗组和对照组各30例。对照组常规骨折切开复位、内固定治疗，治疗组在对照组方法的基础上给予浓缩、富集自体骨髓干细胞移植治疗。比较两组治疗后骨折愈合情况。结果治疗组术后（11．7±5．3）周达到临床愈合标准，短于对照组的（15．9±6．4）周（P＜0．05）。治疗组骨折愈合29例（96．7％），延迟愈合1例（3．3％），无骨不愈合发生；对照组骨折愈合26例（86．7％），延迟愈合3例（10．0％），骨不愈合1例（3．3％），治疗组骨折延迟愈合率低于对照组（P＜0．05）。治疗组均无髂骨穿刺处、骨折处感染和组织坏死发生，但27例骨穿刺处局部出现轻度胀痛，未作处理，3～5 d后症状自行缓解。结论对于骨折患者，在常规手术治疗的基础上给予浓缩、富集自体骨髓干细胞移植，可促进骨折早期愈合，降低骨延迟愈合或不愈合的发生率。

목적：탐토부집자체골수간세포이식연합수술치료골절적효과급대골절술후연지유합혹불유합적료효。방법골절환자60례，안수궤수자표법분위치료조화대조조각30례。대조조상규골절절개복위、내고정치료，치료조재대조조방법적기출상급여농축、부집자체골수간세포이식치료。비교량조치료후골절유합정황。결과치료조술후（11．7±5．3）주체도림상유합표준，단우대조조적（15．9±6．4）주（P＜0．05）。치료조골절유합29례（96．7％），연지유합1례（3．3％），무골불유합발생；대조조골절유합26례（86．7％），연지유합3례（10．0％），골불유합1례（3．3％），치료조골절연지유합솔저우대조조（P＜0．05）。치료조균무가골천자처、골절처감염화조직배사발생，단27례골천자처국부출현경도창통，미작처리，3～5 d후증상자행완해。결론대우골절환자，재상규수술치료적기출상급여농축、부집자체골수간세포이식，가촉진골절조기유합，강저골연지유합혹불유합적발생솔。
Objective To explore the efficacy of bone marrow stem cell ( BMSC ) transplantation combined with conventional operation on fracture and disunited fracture .Methods Sixty cases of fracture were divided into treatment group and control group randomly , with 30 cases in each group .The control group was given conventional operation,the treatment group was given BMSC transplantation combined with conventional operation .A comparison of fracture healing in the two groups was made .Results After operation,the time of fracture healing in the treatment group was less than that in the control group [(11.7 ±5.3) weeks vs (15.9 ±6.4) weeks,P<0.05].In the treatment group,29 cases(96.7%) achieved healing ,1(3.3%) achieved delay healing ,there were no disunited fracture cases .In the control group ,26 cases ( 86.7%) achieved healing ,3 ( 10.0%) achieved delay healing , there was one disunited fracture case.The delay healing rate of treatment group was significantly lower than that of control group (P<0.05). There were no complications ,such as inflammations occurred at the puncture point of iliac and the fracture sites in the treatment group except 27 slightly swollen cases who alleviated 3 to 5 days later .Conclusion High concentration BMSC transplantation combined with conventional operation is effective in the treatment of fracture ,which might improve early healing of fracture as well as reduce the incidence of delay healing or disunited fracture .
