目的：观察颅内压监测对降低重型颅脑外伤患者死亡率的作用。方法300例重型颅脑外伤患者（ GCS评分＜9分）按随机数字表法分为监测组和对照组各150例，两组患者入院后2 d内均给予降颅内压治疗，其中监测组加用颅内压监测，对比两组患者伤后14 d的死亡率。结果监测组患者死亡率低于对照组（P＜0．05），颅内压监测降低重型颅脑外伤患者死亡率（OR＝0．63，95％可信区间：0．41～0．94，P＝0．020）。结论颅内压监测有助于及时发现颅内情况变化及指导治疗，可能改善重型颅脑外伤患者预后。
목적：관찰로내압감측대강저중형로뇌외상환자사망솔적작용。방법300례중형로뇌외상환자（ GCS평분＜9분）안수궤수자표법분위감측조화대조조각150례，량조환자입원후2 d내균급여강로내압치료，기중감측조가용로내압감측，대비량조환자상후14 d적사망솔。결과감측조환자사망솔저우대조조（P＜0．05），로내압감측강저중형로뇌외상환자사망솔（OR＝0．63，95％가신구간：0．41～0．94，P＝0．020）。결론로내압감측유조우급시발현로내정황변화급지도치료，가능개선중형로뇌외상환자예후。
Objective To observe the effects of intracranial pressure ( ICP) monitoring on mortality in patients with severe traumatic brain injury ( TBI ) .Methods Three hundred patients with severe TBI [ Glasgow coma scale (GCS) score<9] were randomly divided into two groups ,monitoring group (150 patients) and control group (150 patients).All patients were treated with ICP-lowering therapies within 2 day in our department of neurosurgery .ICP monitoring was conducted in the monitoring group additionally .The mortalites of two groups were defined at 14 days postinjury.Results The mortality of monitoring group was lower than that of control group (P<0.05),ICP monitoring could reduce the mortality in patients with severe traumatic brain injury (OR=0.63,95%CI:0.41-0.94,P=0.020). Conclusion ICP monitoring helps to discover the change of intracranial situation and guide ICP-directed treatment , which may improve the prognosis of patients with severe traumatic brain injury .
