目的:探讨肺硬化性血管瘤(P S H)的C T特征,以便提高术前对本病的诊断率。方法:回顾性分析11例经过手术或病理证实的P S H,所有病例均具有完整的临床及和C T资料。结果:11例P S H中,男性2例及女性9例,均为单发结节病灶,发生于右肺6例,左肺5例,大小6m m ×8m m~33m m ×47m m,C T平扫均表现为边界清楚、密度均匀的圆形或卵圆形软组织密度影,3例见浅分叶征,2例可见胸膜凹陷征,2例病灶边缘部分见沙粒状钙化,均未见毛刺征,增强扫描呈持续性进行性显著强化,动脉期、实质期CT值比平扫平均增加31HU和62HU,4例PSH动脉期见贴边血管征。结论:肺硬化性血管瘤的CT表现有一定特征性,CT扫描特别是增强检查能为诊断提供重要依据,明确诊断需病理学检查。

Objective:To summarize the CT image features of pulmonary sclerosing hemangioma (PSH), so as to improve the CT diagnostic quality to PSH. Methods: 11 cases w it h PSH proved by surgery and pathology were enrol ed.Results:The CT findings of 11 cases including 2 males and 9females, showed al single lesion, 6 were located in right lobe, 5 located in left lo be, the diameterswere 6mm×8mm~ 33mm×47mm. The CT non- contrast scans of 11 cases demonstrated isolated round or oval soft tissue masseswith clear boundary and homogeneous density, 3 cases with slight lobulation, 2 cases with depressed pleurasigns, 2 cases with calcification like sand inmargin. None of the 11 cases showed spicula sign.The enhanced CT scan showed continuous progressive enhancement and the CT values in arterial phase and parenchymalphase increased for 31HU and 62HU respectively. Blood vessel marginating signs were observed in the arterial phase in 4 cases. Conclusion: The CT image features of PSH have some characteristic and the enhanced CT scan could be helpful in diagnosis of PSH to some extent, but the definite diagnosis should be according to the pathology.
