目的:通过测定小儿体外循环心脏手术的降钙素原(procalcitonin,PCT),观察PCT变化规律并与其它炎症因子相比较,评价PCT对提示小儿体外循环心脏术后早期并发症的临床价值。方法选择检测30例接受体外循环手术的先天性心脏病患儿围术期中几个时期的血液(包括术前、术后即刻、术后4、24、48、72h),并按术后早期是否存在并发症作为分组标准,分别测定各组C反应蛋白和PCT的浓度,对其变化规律进行观察和分析。结果与C反应蛋白比较,降钙素原于体外循环结束后更早、更持续出现于血清中,术后48h可达到峰值,术后72h逐渐下降。术后早期发生并发症患者术后4h、48h、72h的降钙素原浓度高于术后无并发症患儿(p<0.05)。结论降钙素原较C反应蛋白能更早,并持续地在有早期术后并发症的患儿血液中表达。测定PCT有助于临床监测心脏术后高危患儿病情。

Objective This article aimed to explore the change of procalcitonin(PCT) level and the relation between PCT and other cytokines ,and to research the value of PCT to indicate early complication by testing PCT after cardiopulmonary bypass(CPB) operation in infants with congenital. Methods We included in this study 30 patients undergoing cardiac surgery with cardiopulmonary bypass(CPB),who were divided the into two groups according to the severity of SIRS. We determined the concentrations of PCT,CRP in the plasma before operation(T0),post operation(T1),4 、24、48、72hours after surgery(T3、T4、T5、T6),and analyzed the pattern and correlation of their changes. Result After surgery,the PCT level exhibited a significant elevation at the 48h after surgery.Extremely significant from T3 to T6 in the severe SIRS group as compared with the mild SIRS group(p<0.05).Conclusion Compared with CRP ,PCT expresses earlier and persistently in the plasma of the patients with severe SIRS,which suggests that it might be a good predicing marker.
